Issue:
A VA employee self-reported that she lost or misplaced a list of Veterans with personally identifiable information (PII) on it. The list contained the Veterans' name, full SSN and diagnosis.
Outcome:
07/02/13: Further investigation revealed that the information that was on the list included the Veterans' name, SSN and religion. The employee is part of the Chaplain's Service. 07/09/13: The list contained the information on 22 Veterans. The 22 Veterans will receive a letter offering credit protection services.
