Issue:

On March 12, 2014, the VA received a letter from Veteran A informing us that some of his personal information, to include medical records, was released to an address in Louisiana. He further stated that he had never lived in Louisiana and additionally requested a copy of a letter mailed on December 13, 2013. Our records revealed that we mailed a notification letter to an address in Louisiana on December 13, 2013, but no medical records. I called the veteran to inquire about the claim of releasing his medical documents. The veteran informed me that in 2011, his ex-wife called him and stated that she received some of his medical records and that she was returning them to the VA. A further look into his claims file reveals that we mailed duplicate service medical records to the same Louisiana address on August 16, 2011. There is nothing in the file to indicate that the duplicate records were ever returned. I asked the veteran if he had any contact information for his ex-wife. He stated that he has no information on the ex-wife. A letter has been sent to the Louisiana address to inquire about the misdirected documents, no reply has been received as of yet.

Outcome:

05/22/14: The Incident Resolution Team has determined that Veteran A will be sent a letter offering credit protection services.
