Issue:

Veteran A received Veteran B's treatment records in the mail. Veteran A returned the records to his Veteran Service Officer, who in turn mailed to local VBA Outbased Office, who in turn contacted their Regional Office Privacy Officer (PO), who directed them to contact the Orlando Privacy Officer. The records are being mailed from the Orlando Outbased Office to the Orlando Privacy Office for review. Release of Information (ROI) confirms no such request has been handled for either Veteran A or B. Documents total approximately 100 pages. We are presuming that Veteran B's full name, address, SSN, and medical information is embedded in the 100 pages. However, confirmation is expected Friday, 01/03/14 following UPS delivery.

Outcome:

01/08/14: The PO got the records yesterday and it appears that there was a total of approximately 60 pages of protected health information (PHI) containing Veteran B's full name, SSN, DOB, and assorted medical information (i.e. Progress Notes, Consults, C&P exam, etc.). Veteran B will receive a letter offering credit protections services.
