Veteran/Patient A reported to a nursing staff member that he was given a printout of an upcoming appointment, but the printout was for Veteran/Patient B. The printout contained the full name, full social security number, and date of birth of the Veteran/Patient B.

12/02/13: Veteran/Patient B will be sent a letter offering credit protection services due to full SSN being disclosed.
