Issue:
Veteran A came to the Release of Information office to get a copy of his records, but also presented a letter that he had received from our facility. The first page was about his up and coming appointment and the next page he didn't understand. Upon inspection by the Release of Information Clerk, it was found that the second page was another veteran's information with full social security number and full name.
Outcome:
06/27/13: One Veteran will be sent a letter offering credit protection services.
