Issue:

A Veteran was in for his appointment and received a take home lab kit. When he was ready to use the kit, he saw Veteran B's name, full social security number and birth date on the label. He called telephone triage and they sent a message for the nurse to have another kit mailed to the Veteran. They also told him to dispose of the kit. The Privacy Officer spoke with the Veteran 11/26/13 and he stated he was able to pull the label off the tube and he shredded it.

Outcome:

11/27/13: Veteran B will be sent a letter offering credit protection services.
