Pharmacy Service reported the following incident: On January 13, 2014, Veteran A received a medication information sheet in the mail which belonged to Veteran B. No incorrect medications were sent. The incident was detected and reported to Pharmacy on January 27, 2014. Veteran Bs personal identifiable information includes: name, address, providers name, and medication name.
Outcome:
01/28/14: Veteran B will be sent a notification letter.
