Issue:

Pharmacy Service was preparing the medications for Veteran A, and mistakenly placed a medication bottle belonging to Veteran B in the same medication bag. When Veteran A got home, he noticed he received medication for another Veteran and immediately contacted Pharmacy Service. Pharmacy Service sent a UPS envelope for return of the medication. The information of Veteran B included his full name, address and type of medication. No other PHI was on the medication bottle or information sheet.

Outcome:

01/28/14: Veteran B will be sent a notification letter.
