Issue:

A prescription for a VA patient was mailed in error to another Veteran. The actual prescription was processed, filled & bagged for the correct patient on 04/03/15. During the shipping entry, the Pharmacy Technician noted a post-it note on bag which has a different mailing address. The post-it note indicated the package should be mailed to that address. The Technician then entered the address as requested. At this time, Outpatient Pharmacy cannot determine the VAMC who requested mailing address change on the post-it note. The recipient of the prescription notified the Outpatient Pharmacy immediately after receiving the package, stating it was not his prescription. The recipient however opened the package and reviewed the name, address and medication on the prescription. The only sensitive personal information on the package that was inappropriately exposed was the patient's full name, address and name of medication. There was no SSN on the mailing package.

Outcome:

04/10/15: The Incident Resolution Service Team has determined that the Veteran whose information was disclosed will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
