A NC Consolidated Patient Account Center (CPAC) clerk mailed documents to Veteran A. One form (authorizaion form VA 10-5345) belonged to Veteran B and contained his name and address. The form was returend to NCCPAC clerk who notified the Privacy Officer (PO).
Outcome:
01/31/14: Veteran B will be sent a notification letter..
