Patient A requested a copy of the access report for his record. Patient A indicated 13 names of employees who he believes did not have a need to know in regard to accessing his medical record. The report also indicates that Patient As wife, an employee, inappropriately accessed the record without a need to know. (This was also detected on the ISO audit). The record contains 7332 protected information. Update: 03/24/11:Patient A will be sent a letter offering credit protection services.
