Issue:

Veteran A came in to request his medical records. He had called for information on Friday 12/19/14 and was told that he would need to sign an authorization. When Veteran A came in, he brought with him a Disk that had radiology exams on them, but he revealed that these was not his information. The information was sent by mistake to Veteran A. Veteran B's information included his full name, full SSN, DOB and radiology studies. Veteran B's information was supposed to be mailed to a non-VA provider and inadvertently got mailed to Veteran A instead. We have ensured Veteran B's information is now being mailed to appropriate non-VA provider.

Outcome:

12/29/14: The Incident Resolution Service Team has determined that Veteran B will be sent a letter offering credit protection services.
