Issue:

Based on interview and record review the facility failed to ensure that Patient A's protected health information (PHI) was secured during transmission of a bill from the business office. This resulted in a breach of Patient A's PHI when it was inadvertently received by Patient B. Findings:On 4/9/12 at approximately 11:00 AM, an investigation was initiated on an entity reported event of Patient A's PHI being sent to Patient B on 3/26/12.During a phone interview with the corporate compliance specialist on 4/9/12 at 11:30 AM, she stated, "[Used Patient A's name] was a minor. Her bill was sent by business office to Patient B. I notified her mother".A review of the data showed Patient B had received Patient A's paperwork in error and it contained Patient A's PHI that included, "Her name, date of birth, home address, diagnoses and medical record number". Her social security number was not on the misdirected data.This was a breach of Patient A's PHI.
