Issue:
The Privacy Officer (PO) received a phone call from a Veteran's daughter informing that her father received a packet of medical records that belong to Patient B. Release of Information (ROI) will send Patient B's medical records to the correct address/requestor. After researching this, the PO found the following:1. Release of Information received a request from a third party (Law firm) on 6-15-12 requesting the medical records on Patient B. 2. On 6-19-12, the records were inadvertently sent to the Veteran with the same last name of the Law Firm (wrong address chosen). Update: 06/26/12:Patient B will be sent a letter offering credit protection services due to full name and full SSN being disclosed.

Clerk was counseled.
