Issue:
Veteran A who is a patient in one of our facility's domiciliary was given an insulin shot that had Veteran B's name on it. No other information was disclosed to Veteran A. The Nurse immediately took the incorrect shot from Veteran A to retrieve the correct one.
Outcome:
08/15/13: Veteran B will be sent a notification letter.
