Issue:

Employee inadvertently uploaded a patient's home telehealth technology waiver form onto the VISN SharePoint site. The form contained the patient's first and last name and the full SSN. Approximately 290 VA VISN employees have access to the SharePoint site, and staff include health technicians, nurses, LPNs, clinical coordinators, and medical support assistants.

Outcome:

10/22/14: The Incident Resolution Service Team has determined that there was a policy violation. While there was an unauthorized disclosure of data, it has been determined that the incident has a low probability of a risk of compromise.
