Issue:
A Release of Information (ROI) request was submitted to the Orlando VAMC outpatient clinic for processing. The clerk accidentally pulled Veteran B's medical records and mailed to the requester. Upon receipt of the records, the requester noted it belongs to another Veteran. He then called to notify the outpatient clinic about the error and also sent back the records. The records contained the other Veteran's full name, SSN and DOB.
Outcome:
01/30/15: The Incident Resolution Service Team has determined that Veteran B will be sent a letter offering credit protection services.
