Issue:
Upon review of a Sensitive Patient Access Report (SPAR), the Privacy Officer (PO) found a Provider accessed a Veteran's records and states that if she accessed them, could have been an error because she had the wrong last 4 digits of the SSN. Once she realized it she would automatically got out of the record. It could also be that she received an alert for coverage of another provider and again if she saw that it was not her patient she would exit immediately.
Outcome:
04/15/15: The Incident Resolution Service Team has determined that there was a privacy violation, but no data breach has occurred, as this was done in error. No malicious intent was invovled.
