Issue:

A Veteran's care packet was inadvertently faxed to the wrong vendor on April 8. The error was discovered within 10 minutes and the vendor was instructed to shred the packet. Next morning the Veteran's care packet was faxed to the correct vendor and confirmed receipt that afternoon. On April 17, the Veteran was contacted by first vendor for scheduling appointment. Care manage re-contacted the first vendor's staff and again directed them to shred the mis-faxed information from previous week. Care packet contained Veteran's full name, full SSN, DOB, home address, and surgical consult information. Subsequent review revealed that the fax error was caused by using first vendor's fax cover sheet as a templtate for the intended vendor, however staff failed to change the fax number.

Outcome:

04/22/14: The Incident Resolution Team has determined that no data breach has occurred. The document was seen only by a HIPAA covered entity.
