Issue:
On Saturday 10/26/13, an appointment list was printed to a printer where Veterans had access to it without a staff member being in the room. This is a telephone clinic and the patient is here in Poplar Bluff and the provider is in Cape Girardeau. The provider was in Poplar Bluff on Friday 10/25/13 and saw patients and apparently she didn't change her printer when she got back to Cape and printed her appointment list in the room where the patients are seen for their telehealth appointment. Two appointment lists were printed to the machine and all were still on the machine, but the Veterans had access to see them while alone in the room.
Outcome:
11/06/13: A provider found the information face down on the printer, it was printed 5 days earlier. There are no cameras in the area. The information did not look like it was disturbed but there is no real way to tell since it was on the printer that many days. The appointment list contained 8 Veterans appointment information, full name, and home phone and address. Due to the time frame and uncertainty of who may have seen the information, eight (8) Veterans will receive a notification letter.
