Federal health inspectors from the Centers for Disease Control and Prevention inspect cruise ships twice a year, checking everything from kitchen equipment to dishwasher temperatures, and grade ships based on what they find. A score below 86 is considered failing. Cruise ships are also required to report all illness outbreaks affecting more than 3% of passengers or crew to the CDC. Inspections and outbreaks from 2010 to the present are shown below.

Water was leaking from the top of the reduced pressure assembly backflow prevention device by one of the large bolts. Crew tightened the bolt and the water leak stopped.

The ship had multiple occasions where the bunkering process started and stopped throughout the time the bunker line was connected to the shore connection. When this occurred the bunker pretest of the shore side was not documented. For example, bunkering started on 1 March at 7:05 and the initial chlorine and pH values were properly recorded. Bunkering then stopped at 13:30 and restarted at 16:20. No shore side chlorine or pH values were taken at the restart. Bunkering continued until 21:25, stopped, and then restarted again at 1:45 on 2 March. No shore side chlorine or pH values were taken at the restart This bunkering segment stopped at 8:05 and restarted again at 14:40. No shore side chlorine or pH values were taken at the restart.

Potable Water-Potable Water Tank Maintenance

Touch-up paint was applied to potable water tanks 2P, 2S, 3P, 3S, and 5P when they were inspected since the beginning of 2015 but it was not documented if the paint was applied and dried in accordance with the manufacturer's recommendations.

Other-Deck 10 Men's Changing Room and Sauna

No handwash sign or pictograph was posted at the handwash sink located inside the handicap accessible stall.

Four bulkhead plugs were missing, exposing holes. This was corrected.

A red bucket in the worker side of the buffet was had an Oxivir Five16 label affixed to the outside of the bucket. It was determined that the liquid in the bucket was soapy water and not Oxivir. The Oxivir label was removed immediately.

Bar-Staff

The bulkhead-mounted menu board above the back bar counter was made of a soft cloth material and was not hard or nonabsorbent.

Pantry-Staff

Two slotted fasteners were in the food-contact zone of the coffee strainer on the espresso machine.

Medical-Acute Gastroenteritis (AGE) Surveillance Log

The AGE log for the cruise dated 14-29 April generated by the electronic medical database system had incorrect total passenger and total crew numbers. The log indicated there were 2185 passengers and 953 crew, when there were actually 2121 passengers and 926 crew. The AGE log for the cruise dated 29 April to 2 May generated by the electronic medical database system had incorrect total passenger numbers. The log indicated there were 2079 passengers, when there were actually 1975.

Medical-Acute Gastroenteritis Illness Reports

Several of the AGE reports submitted on 27, 28, and 29 April had incorrect total passenger and total crew numbers. A report on 29 April indicated there were 2185 passengers and 953 crew, and reports on 27 and 28 April indicated there were 2182 passengers and 953 crew. There were actually 2121 passengers and 926 crew. The AGE report submitted on 30 April had incorrect total passenger numbers. The report indicated there were 2078 passengers. There were actually 1975 passengers. These incorrect reports were generated by the electronic medical database system. The medical staff recognized the errors and submitted separate reports with the correct numbers.

Ventilation-Fan Room 430

The condensate drain pan for air handling unit 410 was not able to be viewed during the inspection because it was not able to be opened. According to the staff, it would take several hours to open. This was written on the previous inspection. A plan had been developed to install a sight glass for viewing the condensate drain pan during the 2015 dry dock.

Pantry-Deck 7 Aft

There were paper napkins instead of paper towels at the handwashing station. When the inspector used the napkins, small pieces stuck to the hands.

Medical-Crew Acute Gastroenteritis (AGE) Reporting and Isolation

A nonfood worker had an AGE illness onset at 7:30 am on November 17 and reported to medical at 11:17 am on the same day. According to their work record, this individual worked from 8:21 am until 9:58 am on November 17 before reporting to medical.

Medical-Crew AGE Reporting and Isolation

According to the AGE surveillance log, one food worker had an illness onset on October 23 at 7:00 pm of two episodes of diarrhea and no other symptoms, but they did not report to medical until 10:00 am on October 24. The medical documentation also indicated this individual had stomach cramps. According to the medical staff, they were not sure if this individual's diarrhea symptoms were above normal or not. The medical records indicated this individual was released from isolation at 7:45 am on October 25, but their documented last symptom was at 9:00 pm on October 23. According to their work record, this individual worked from 5:30 pm until 10:30 pm on October 25.

According to the acute gastroenteritis (AGE) surveillance log, one food worker had an illness onset on October 23 at 7:00 pm of two episodes of diarrhea and no other symptoms, but according to additional medical documentation, this individual also had abdominal cramps.

During the cruise from November 7 - 22, 2013, a passenger had two episodes of diarrhea, was given antidiarrheal medication and was documented as nonreportable. 'None' was written in the underlying illness column, but the medical database stated that this person was lactose intolerant and that was what caused their symptoms.

Potable Water-Cross-Connection Control Log

The cross-connection control log stated the mineralizer had four backflow prevention devices and four air gaps. Upon inspection, there were four air gaps on the drain lines for each of the four mineralizers and one backflow prevention device on the potable water supply line to the mineralizers. On the air gap list, four air gaps were listed for the buffer tank in the deck 9 pool machinery room, but there were also two backflow prevention devices listed for the same plumbing connection. According to staff, there were no air gaps for this buffer tank. Also, there were two air gaps listed for the 'sanitary fresh water section' and there were two backflow prevention devices listed for these same areas. According to staff, these devices and air gaps were for the analyzers for bunkering and production. On the cross-connection list, this same area was indicated as the technical water chlorine station. Upon inspection, there were no air gaps for the production sample lines in pump room 5 starboard and there were two backflow prevention devices, one for the lower loop and one for the upper loop. Also, the backflow prevention devices were heavily corroded. There was a backflow prevention device for the deck 10 Persian Garden fountain in the spa area, but upon inspection this fountain was no longer used and staff confirmed the plumbing components had been disconnected. Also, the cross-connection log indicated a backflow prevention device on the potable water line to the Hi Fog line, but upon inspection, only technical water was connected to this line. Finally, four backflow prevention devices were listed to be outside of the deck 9 cabin numbers 9098 and 9096 and on deck 8 outside cabins 8104 and 8106. The log stated these devices were for a Jacuzzi, but staff did not know what the devices were for and it was not clear if the devices were for a private whirlpool spa or if it was for individual tubs inside of each cabin. Upon inspection, it was confirmed that these devices were on the hot and cold water lines of the individual tubs in each of the cabins.

Potable Water-Striping

The potable water line from the reverse osmosis system was not striped blue or blue/green/blue every 15 feet after the chlorine injection point. Also, the permeate line from the reverse osmosis system to the chlorine injection point was not striped blue/gray/blue every 15 feet. Finally, the potable water line in the deck 3 fan room #430 was not striped for potable water.

Other-Bridge

There were approximately 10 boxes of 12 bottles of water each stored on the carpeted deck on the bridge. Also, there was no coving at the bulkhead/deck juncture.

Recreational Water Facilities-Antientrapment Drain Covers

The gravity emergency drain covers in the aft and forward swimming pools and the hydropool were not properly installed according to the manufacturer's instructions. Upon inspection, there was a large gap between each of these drain covers and the surfaces of the pools that could cause a potential entrapment hazard.

Integrated Pest Management-Active Surveillance Inspections

Active surveillance inspections were conducted in the garbage handling area on November 19 and 21, but there were no inspection times recorded.

Recreational Water Facilities-Filter Housing Sanitation

On October 9, the sand for the forward swimming pool sand filter was replaced, but there was no documentation that the filter housing was cleaned and sanitized before replacing the sand. According to the staff, the housing was cleaned and disinfected with a 200 ppm chlorine solution.

Buffet-Seaview Cafe - Beverage Station

Clean beverage glasses were stored wet with no way for excess water to drain when inverted onto a plastic tray.

Dining Room-Seaview Cafe - Waiter Station

Two spray bottles of disinfectant used for nonfood contact were stored on the top of the two different waiter stations in the soiled area, rather than in the cabinets marked for their storage.

Buffet-Seaview Cafe - Beverage Station

A large amount of slime mold was noted in and around the ice bin where the ice was discharged from the counter-mounted ice maker into the bin from the clear vinyl hose.

A large plastic lined platter in clean storage had a grease film on it.

The bowls were not loaded into the glasswash machine properly and some of the bowls were not able to stay in a position that exposed the bowls to the sprays from all the cycles in the glasswash machine. The light intensity behind the counter-mounted combination steamer oven was less than 110 lux. Paperwork was shown to the inspector including where the light had been ordered and that it was due for pickup at the end of the cruise.

Flow meters were not installed for any recreational water facility. Staff stated flow meters for the whirlpools have been ordered and flow meters for the swimming pools will be ordered shortly following the inspection. All flow meters will be delivered on 7 November 2012. The turnover rates for all recreational water facilities will be calculated once the flow meters have been installed.

Recreational Water Facilities-Whirlpool Safety Signs

The safety signs for the whirlpools cautioned against individuals with high blood pressure, not high or low blood pressure.

Potable Water-Air Gaps

The list of air gaps was last updated in September 2011. This list contained the same items as the list of testable backflow prevention devices, and no other air gaps were identified.

Potable Water-Mineralizer

The air gap for the mineralizer backwash drain was not included in the list of cross-connections.

Children Area-

In the toilet room for children five years and below, the height of the toilet was measured at greater than 280 millimeters (11 inches) and there was no step stool immediately available. This area had one step stool for both the toilet and the sink, which was also higher than the required height of 560 millimeters (22 inches).

The combined whirlpools had only one chart recorder for automated monitoring instead of one for each individual whirlpool.

Throughout the food areas there were a number of pieces of food preparation equipment that were labeled as 'Not In Use' that create additional work for cleaning. These included the combination steamer oven and blast chiller in the vegetable preparation room, the thawing microwave oven in the butcher shop, a hot holding cabinet in the Officer's Mess, and pizza ovens in the lido.

Food Service General-Light Levels

The light level was less than 110 lux around and behind deck-mounted equipment, including the combination ovens and ice machines.

A heavy layer of mildew and dust was on the top of the back evaporator and coating the heat transfer fins of this unit.

The plastic inner wrap and outer carton top were both open on a box of stored almonds.

The final sanitizing rinse water had just been changed and a water temperature of 163°F was measured immediately after a clean pot had been removed from the sanitizing water. The pot was returned to the sanitizing water after the temperature reached 171°F.

The consumer advisory specified eggs as the food item of concern on the placard at this outlet rather than raw fish. In addition, the consumer advisory at the Pool Grill specified eggs as the food item of concern rather than hamburgers.

A small fly was behind the counter in the open service area.

The sanitizing solution was heavily soiled with food debris. Along with the wiping cloth, a green plastic scrubbing pad was also stored in the solution.

Medical-Acute Gastroenteritis (AGE) Case Definition

The written AGE case definition used by the ship did not consider the number of diarrhea episodes that was above normal to certain individuals. This definition did not allow individuals with pre-existing medical conditions, such as irritable bowel syndrome, who reach 3 loose stools to be considered non-reportable cases. No cases appeared to be reported incorrectly.

Medical-Asymptomatic Cabin Mates of Symptomatic Crew

The date and time of verbal interviews for cabin mates of symptomatic crew members was not always documented accurately. There were instances in the logs that the date and time for the follow up interviews at 0 hours, 24 hours, and 48 hours were the same.

Potable Water-Microbiological Monitoring

The ship used Colisure for coliform tests on water samples from bunkering and distribution. The incubator was not operated in accordance with the reagent manufacturer specifications. On the day of the inspection, several samples were incubated at a temperature of 37°C when the Colisure manufacturer's specified an incubation temperature of 35 +- 0.5°C. An internal document was presented to the inspector where per company's policy, the incubation temperature was allowed to be between 35°C and 38°C but not less than 34.5°C, which was different from the reagent manufacturer's specifications.

Potable Water-Bunkering & Production Records

An instruction on bunkering and production records specified an acceptable range of pH levels between 7 and 8.

Potable Water-Distillate & Permeate Water Lines

The distillate water lines from the evaporators to the potable water halogenation station and the permeate water lines from the reverse osmosis plant to the potable water halogenation station were not striped or painted in accordance with ISO 14726 standards (blue/gray/blue).

Potable Water-Tank Maintenance

Tanks #3 port and #3 starboard underwent maintenance on 14 November 2011 and tank #6 center on 2 November 2011. There were notations that the tanks were super chlorinated and then they were put back in service. However, there was no written documentation on the free halogen residual level before the tanks were put back in service. In addition, they were touched-up with an approved coating system but there was no written documentation that the manufacturers' recommendations were followed for application, drying, and curing. Also, there was no written documentation of approval from a certification organization independent of the coating manufacturer.

The pH and halogen analyzer charts had no initials.

The OPRP did not include procedures to minimize respiratory and dermal exposures to both passengers and crew. During the inspection, a housekeeping staff member was fogging in the vicinity area of the Cafe al Bacio. The bar manager, the bartender, the VSP inspector, and a group of violinists performing at the time were exposed to the fogging, making them cough after breathing the disinfectant mist.

The final rinse temperature gauge on the in-use flight-type dishwash indicated temperatures of 200-220°F. The temperature at the plate surface was 170°F. Also, the prewash temperature gauge was reading below 100°F and was measured with the inspector's thermometer at 140-150°F.

During the active use of a tilting pan and while a staff member was opening the lid, a knob used to release the tilting pan lid was loose and fell into the beef stock. The crew members were instructed to take the knob out of the food.

There were gaps and complete openings to void spaces in the technical compartments of multiple reach-in and undercounter refrigerators. These gaps and openings to void spaces were located where power cords and refrigeration pipes penetrated bulkheads and deckheads. This was observed in different areas, such as the mini galley hot section of the Lido galley, Pool Grill pantry, Lido ice cream station, Pool Bar pantry, and Cova Cafe.

He added that this explanation was described in a manufacturer's certificate that he couldn't find at the time of the inspection.

The potable water lines to the sink had non-continuous pressure type backflow prevention devices. There was a valve downstream from the backflow prevention device. The cross-connection control program indicated that Watts N9 devices were attached and they were inspected since September 2010.

The cross connection control program indicated there was an Oras device attached to the potable water lines for the pedicure stations, but Watts N9 devices were connected to the lines. The program also indicated a total of three devices for the hair sinks, but there were six devices installed, two at each sink. The program listing indicated that both of these had been inspected since September 2010.

Potable Water-Lower Loop Analyzer

There was greater than 0.2 ppm difference between the chlorine level indicated on the analyzer and the manual test result by a crew member and the inspector. The analyzer ranged between 0.74 ppm and 0.78 ppm, and the manual test results ranged from 0.35 ppm to 0.52 ppm. The analyzer was recalibrated.

Potable Water-Tank Maintenance

Maintenance, including painting, was conducted on the #2 port tank on 5-6 November 2010. There was no indication in the records that the manufacturer's guidelines for drying/curing of the paint was followed prior to refilling the tanks with potable water.

The condensation drain pan for the air handling unit "AHU Supply Section AW 715" was not easily assessable for inspection. There were only small holes along a panel on one edge of the pan.

Fire Fighting

Observed during the general walk through the spaces in the machinery spaces & passageways in between, several portable fire extinguishers were missing, bracket mounts broken, & some misplaced; EDG room 01-21, back stage hallway 3-01, aft I-95 passageway Port side, & machinery space mid-section. Also, several fire hoses in machinery spaces were found disconnected from Main Valve including nozzles.

Accommodation/Occupational Safety

Crew laundry spaces exhibited signs of lack of upkeep to prevent fire. Crew laundry spaces have excess lint and ducts are not connected. Vent systems should be connected and free of excess lint to prevent the start/spread of fire. Issued Form B.

Accommodation/Occupational Safety

During a fire drill in the back stage entertainer dressing room, one overhead lighting (fluorescent lamp) missing a safety guard cover.

Operations/Management

During the spot check of the transitional batteries, found the spare key in the box for the space missing. No reasons given by the licensed engineers.

Material Failure (Vessels)

The vessel CELEBRITY INFINITY was in the process of raising the falls after a lifeboat launching when the wire falls failed to stop. This caused the blocks to contact the davit and put the wire under tension. The wire eventually broke and the block fell to the water. The vessel is without a means to recover their life boat.

On July 17, 2012, at approximately 0800 Sector Juneau received a call reporting that the M/V CELEBRITY INFINITY, operated by Celebrity Cruises Inc., discharged an estimated 0.25 gallons of hydraulic lube oil into Gastineau Channel, a navigable waterway of the United States, in a harmful quantity in that it created a sheen on the water's surface.

The passenger vessel CELEBRITY INFINITY reported a discharge of approximately 0.1 liters of hydraulic fluid due to a ruptured hose. Vessel personnel secured the hose and cleaned up the discharge with sorbent pads.
