Federal health inspectors from the Centers for Disease Control and Prevention inspect cruise ships twice a year, checking everything from kitchen equipment to dishwasher temperatures, and grade ships based on what they find. A score below 86 is considered failing. Cruise ships are also required to report all illness outbreaks affecting more than 3% of passengers or crew to the CDC. Inspections and outbreaks from 2010 to the present are shown below.

Preparation Room-Deck 2 - Poultry Preparation
Water was dripping from three locations on a center deckhead seam. The water was falling onto the main prep table in this prep room. The area was not in operation and no food was impacted.
Provisions-Deck 2 - Meat Freezer
The leak described in the first instance in violation 2 above, had pooled onto four cartons of veal and dripped down the sides of these cartons.

Water had accumulated in the deckhead and leaked around the right deckhead hatch and frame. Icicles had formed that were at least 10-inches long from this area. The hatch to the left also had icicles. The technical team examined the problem and determined that a exhaust ventilation pipe had a bad gasket allowing the moisture to drip onto the area above this hatch. Repairs were started immediately.
Preparation Room-Deck 2 - Potato Room
The left potato peeler's door gasket was in extremely bad repair and difficult to clean. The ship replaced the door gasket while the inspection team was onboard.
Preparation Room-Deck 2 - Potato Room
Old potato residue was found in the cracks in the gasket noted above. Dried soil was also noted in the vegetable processor that was stored as clean in the adjacent vegetable preparation area.
Galley-Deck 3 - Cold Pantry
A wiping cloth sanitizer solution was heavily soiled with grease.

Sticker residue with a slight accumulation of food soil in it was found on a cutting board.
Medical-Crew Gastrointestinal (GI) Illness Reporting
At 3:30 pm on 6 September a food area utility cleaner experienced GI illness symptoms. This staff member ate dinner in the crew mess at 6 pm and reported to the medical center at 7 pm. At 10 am on 10 September an engineer experienced GI illness symptom onset during a work break. This staff member went back to work at 10:30 am and reported to the medical center at 2 pm. Both crew members were disciplined.
Children Area-Fun Factory
The water temperature at the sensored-faucet for the handwash sink in the child toilet reached temperatures over 120 °F when adjusted to the hottest setting.
Recreational Water Facilities-Whirlpool Spa Records
The maintenance portion of the whirlpool spa log records specified for week 1 and week 2 the cleaning and disinfection of the hair and lint strainers, but did not note any cleaning or disinfection for the hair and lint strainer housings. The pool maintenance log did specify both the strainer and strainer housings. Staff were questioned about the work performed and did state that the housings for the hair and lint strainers were cleaned and disinfected when the strainers were removed for cleaning.
Pantry-Pool Bar Pantry
There was a live mosquito on the handwash station side panel of the ice and water dispenser, near the pantry entrance. The insect was destroyed. A live fruit fly was observed flying in the area of the dishwash soiled landing counter.
Galley-Deck 3 / 4 - Main Galley - Dining Room Entrance
The deck was worn at the entrance to the galley's from the dining rooms creating a difficult to clean area.
Galley-Deck 5 - Bistro on Five - Dishwashing
The sanitizing solution in the three compartment sink was measured at less than 50 PPM free chlorine residual. Additional chlorine was added to bring the solution up to 100 PPM.
Galley-Deck 5 - Cafe El Bachio
Two plastic cartons of milk were found at 46° and 47°F in the row closest to the door of the reach-in cooler. A carton was checked from the middle of the cooler was found at 37°F. The out of temperature milk was taken to a blast chiller.

On the right flat grill, the grease tray housing was soiled with food debris and standing water. The grill and housing had been previously cleaned.

The back plate of the previously cleaned food slicer was soiled with food debris. When moved, colored liquid poured from the slicer. A crew member immediately began cleaning the slicer.

The cooling logs were not completed for boiled eggs on 28 June. No temperatures were written at 10:57 and 12:25. The log was initialed by the person responsible.

One live fly was near the small elevator. The area was in operation during the inspection.
Buffet-Oceanview Cafe - Stir Fry Station
To the left of the oven, the middle bulkhead panel was misaligned. The right side of this panel was spot welded, exposing seams. There was a large hole at the lower area of the panel, leading into a void space. Staff stated the wires for the newly installed flat grill were being replaced.
Galley-Oceanview Cafe - Potwash Area
A broom and dustpan were stored near the soiled storage. The area was in operation during the inspection. These items were moved to the cleaning locker.
Buffet-Oceanview Cafe - Line 2 Port Aft
The light intensity was less than 220 lux at the flat grill and preparation counter.
Medical-Food Worker Reporting
A cook became ill with acute gastroenteritis symptoms at 22:30 on 22 June 2014. After experiencing symptoms, contrary to the ship's policies and training and the VSP Operations Manual, the worker reported to work without informing the supervisor of the illness. This worker did not report the illness to the medical center until 09:39 on 23 June 2014. This worker was immediately terminated from employment.
Bar-Deck 4 - Martini Bar
The bar was in operation during the inspection. The normal operating lights only provided 50 lux at the bar counter's handwashing sink. The lights could be adjusted to a brighter illumination which was demonstrated to the inspection team.
*General Comment-Commendation
The Celebrity Solstice's teams managing their medical center, potable water, recreational water, housekeeping, HVAC, IPM, child activity center and food safety programs deserve special recognition and commendation for their outstanding compliance with the USPHS/ VSP Operations Manual observed during today's detailed operational inspection.

Medical-Acute Gastroenteritis (AGE) Log
The underlying illness column was left blank for 6 cases during the voyage from 9/23-10/4 and for 5 cases during the voyage from 9/13-9/23.
Medical-Electronic Reporting System
The patient case file and the summary page did not always match regarding isolation times. For example, a crew member's file stated the last symptom was on 9/30 at 17:30 and he/she was released from isolation on 10/2 at 17:30. This crew member was a food worker and met the 48-hour isolation requirement. However, the summary page for this case indicated the isolation ended on 10/2 at 7:29, which is less than 48-hours from last symptom. Medical staff was aware of this issue and had already contacted corporate to correct.
Medical-Acute Gastroenteritis (AGE) Log
A test case was entered into the AGE log for the voyage from 9/13-9/23 by the corporate office. This case was never removed from the AGE log and was counted as a reportable case in the submitted 24 hour report. The AGE log listed 17 ill passengers for the voyage when the total was actually 16.
Medical-Acute Gastroenteritis (AGE) Log
A passenger reported to medical on 9/3 with one loose stool and informed staff this was above normal for him. 'Above normal for individual' was listed in the underlying illness column. This case was listed as nonreportable on the AGE log for the voyage.
Potable Water-Portside Midship Bunker Station
The bottom fill line was not capped during the inspection. No bunkering was taking place at the time.
Integrated Pest Management-Rat Guards
The rat guards on the aft mooring lines were not properly installed and were ineffective. One rat guard was used to cover multiple mooring lines which left multiple openings were rats could enter. Other mooring lines did not have any rat guards.
Buffet-Lido Starboard Waffle Station
The preparation area handwashing sink automatic water valve control would not activate when the inspection team entered the area. The sink started working immediately when the engineer examined it. Another handwashing sink was available around the corner in the pizza preparation station. A crew member at the waffle station was using the adjacent preparation sink for handwashing. No food preparation activities which required the use of this sink were occurring at this time.
Galley-Dining Room Entrances
The deck was worn and difficult to clean in the port and starboard passage ways between the dining room and the galley.
Galley-Pot Washing
A large wire beater, a large mixing bowl and a large pot in the clean utensil storage area had a small amount of dry food soil on them.
Galley-Silk Harvest / Tuscan Grill
One bowl in the clean utensil storage area was found soiled with old food debris.
Galley-Cold Galley
A cleaned meat slicer had a small amount of dried food debris on lower side of the backplate. The slicer was immediately recleaned.
Bar-Coffee Bar
One coffee cup and one saucer in the clean utensil storage area had dried food soil in them.
Buffet-Lido Pizza Station
Standing water and food soil was noted in the deck recess of the fire shutter door channels.
Food Service General-Juice Machines in Lido and Other Beverage Stations
The newer style juice machines had old juice residue in most of the small tight areas to the right and left of the closed technical compartments at the front of the machine.
Bar-Coffee Bar
In the clean utensil storage area in the pantry, wet coffee cups were found nested together in a manner that precluded effective air drying.

Galley-Bakey (Deck 4)
A trolley had a difficult to clean worn rubber seal on the door. Seal was removed and replaced immediately by the ship staff.
Galley-Pizza
The posted time control plan specified that color dots were used for labeling food being held under the ship's time control procedures. The operation had shifted to a set-up time and discard time label for the pizza ingredients and finished pizzas being held for service to passengers. The old plan was removed and changed to reflect the current practice being used in this area.
Integrated Pest Management-Records
The inspections of the incoming food supplies between 6/21-8/23 could not be found in the ship's integrated pest management records. Deck and hotel supplies delivered weekly on these dates were in the binder. The missing records were later delivered to the inspection team.
Medical-Crew Reporting
A nonfood worker had four vomiting episodes, muscle aches, and a headache on July 26, 2013 at 2030, but did not report until July 27 at 0840. According to the documentation, this individual did not work while sick.
Medical-Close Contacts
A nonfood worker had five episodes of diarrhea and vomiting on August 3 and was documented as a reportable case, but there were no close contact interviews recorded. According to the staff, this individual did not share a room and did not have any close contacts.
Medical-Acute Gastroenteritis (AGE) Surveillance Log
On August 6, a nonfood worker had two episodes of diarrhea and stomach cramps, but was recorded as a nonreportable case on the AGE log. This individual was given antidiarrheal medication and was isolated for 24 hours. The underlying illness column on the AGE surveillance log stated 'nil of note'. The medical records indicated one loose stool for this individual was normal, but according to staff, two loose stools was above normal for this individual.
Galley-Deck-mounted Mixer
There was one slotted fastener in the food-splash zone.
Galley-Middle Prep Counter
There was an insect light trap partially over the middle preparation counter to the right of the handwashing station. According to staff, a work order had been submitted to remove this device.
Buffet-Crew Mess Line1 Beverage Station
The inside of the in-use water tubing of the Vitality juice machine was soiled with black residue.
Buffet-Crew Mess Line1 Sunset Diner Cold Trolley
According to staff, this unit was on time control, but was not labeled for time control and was not listed on the time control plan. No food was in the trolley during the inspection.
Buffet-Crew Mess Line1, 2, and 3
There were two separate undercounter refrigerators in each of these lines and only one of the two refrigerators in each area had a label. The time control plans for these areas only listed the one number for two units on each line, but according to the technical staff, all of the undercounter refrigerators were separate units. All of the units were labeled for time control.
Buffet-Crew Mess Bistro 95
There was a yellow sticky residue on the left side of the food-splash panel of the Vitality juice machine and the inside of three of the four dispensing nozzles were soiled with food residue. The unit was not in-use, but was previously cleaned.
Buffet-Crew Mess Bistro Line 1
There was no serving utensil for the container of salami on the self-service buffet line. The line was open during the inspection.
Children Area-Toy Cleaning
The small toys and pieces of toys that are cleaned in the dishwashers were very clean. The larger toys that are cleaned by wiping have many areas where soil and dust are accumulating in the crevices and rough surfaces, such as between and around the letters where the alphabet letters are embossed and slider pieces are mounted.

There was a consumer advisory on the menu but salmon caviar was not cross-referenced to the advisory. All other animal-derived foods that could be consumed raw or undercooked were identified with asterisks.

A cook actively preparing ready-to-eat caprese salads had a partial beard and was not wearing a facial hair restraint. This was corrected during the inspection.

A trolley used for cheese serving had rough surfaces, crevices, and exposed raw wood, making cleaning difficult.
Galley-Deck 3 & 4
There was missing and recessed deck tile grout in many areas of the two decks of the main galleys. There was pooled water between tiles in many areas. There were also many previously cleaned and not in-use areas where it was not clean between tiles.
Galley-Deck 3 - Port Coffee Pantry
The milk tube of the previously cleaned and not in-use espresso machine was soiled with milk residue. The staff sent the tube to the scullery.
Galley-Deck 3 - Starboard Entrance
The spout of the water fountain was heavily soiled with dark residues. This was corrected during the inspection.
Bar-Pool
There was ice dumped inside the handwashing sink.
Preparation Room-
There were 2 out of 3 cutting boards on the clean racks that were scratched and scored, making cleaning difficult.
Medical-Crew Acute Gastroenteritis (AGE) Reporting
An audio visual apprentice crew member had an AGE symptom onset at 10:30 am on 1 May and reported to the medical center at 5:24 pm the same day. This crew member worked from 10:24 am to 3:59 pm according to the work history file.
Medical-Acute Gastroenteritis (AGE) Surveillance
On the 8-25 April sailing there were several crew members who first presented to the medical center with AGE symptoms, such as diarrhea, who were not declared as AGE cases. When those crew members returned to the medical center for follow-up consultations they had additional symptoms. There was no follow-up comment describing the type of symptoms being experienced and there was no way to change the status of these individuals to reportable cases in this follow-up screen for the electronic medical system used. Staff could only add the symptoms to what was the initial presentation screen, even though these additional symptoms did not occur on the initial presentation. The crew cases with one or two diarrheal episodes who had further symptoms and remained non-reportable in the reporting database included two waiters, one assistant waiter, one bar server, and one assistant stateroom steward.

The power cable and water supply tube to the espresso machine was draped on the counter, making cleaning difficult. This was corrected during the inspection.

The potable water tank maintenance records indicated that 6 different tanks were spot painted between September and October of 2012 and there was no notation that the manufacturer's specifications for application of the coatings were followed. The drying and curing times were provided and they met manufacturer's specifications.

The three curved chopper cutting blades stored in a hotel pan on the upper counter shelf were severely damaged, making cleaning difficult.

The water temperature from the handwash station at the clean end of the conveyor dishwash machine was 129 °F. This was a sensor activated sink with no user control of temperature.

Seven live fruit flies were observed on the deckhead over the clean landing counter for the hot-water sanitizing sink and opposite that sink. The area was in full operation at the time of inspection.

During use of the conveyor pot wash machine no water was observed spraying from the final rinse spray nozzles (upper and lower). The display on the conveyor machine was noting an error under the hazard triangle symbol with a red light flashing to indicate some condition, but staff were not able to identify that condition when asked. Later the display described this error code as 316 final rinse insufficient water. Staff repaired the machine during the inspection.

The supply air ventilation cover at the deckhead directly above the conveyor potwash machine clean landing table was heavily soiled with accumulated black mold and dirt residue trapped between the metal filter above the deckhead and the vent cover at the deckhead.

Two live fruit flies were noted on the deckhead near the entrance to the bakery from the pot wash side. The area was in active use at the time of inspection.

Medical-Acute Gastroenteritis (AGE) Case Definition
The AGE case definition used by the ship did not consider the number of diarrhea episodes that was above normal to certain individuals. The current definition did not allow individuals with pre-existing medical conditions, such as individuals with irritable bowel syndrome, who reach 3 loose stools to be considered non-reportable cases. The dual drains for the swimming pools were more than 3 feet apart, but it was unclear if the drain covers were antientrapment drain covers.

The counter mounted ice machines at all the beverage stations were not sealed flush to the counter which presented open seams that prevented proper cleaning. Silicone sealant had been used to close the seams but in several areas on all the ice machines it has since peeled or broke apart.

The undercounter warewash machines in the preparation rooms had been out of order for approximately one month. According to the staff, they were waiting for spare parts. Items from these areas were taken to the main galley for washing.

There were unsealed refrigeration line penetrations in the bulkhead sides of undercounter technical compartments R-02-14A and R-02-14B. This was corrected.

The deck was soiled in the back right corner below the technical compartment for the ice well on the orange service line. This was corrected.

There were two slotted fasteners in the upper interior of the dispensing chutes on the ice and water dispensing machines in the beverage stations. Some of these fasteners were corroding.

It could not be determined if the microwave in the crew mess was of commercial grade and met accredited food equipment sanitation standards for materials, design, and construction. The oven had a label inside stating "household microwave oven." According to the staff, this unit had been purchased from a commercial restaurant supply company. The supply company was contacted for documentation.

Galley-Deck 3 - Starboard Hot Galley
The vent on one of the backflow prevention devices was plugged. Once the plug was removed, the backflow prevention device was leaking continuously from the vent. The device was replaced.
Galley-Deck 3 - Roasting Station
There a pan of cut raw broccoli in one of the reach-in refrigerators. There was a black substance on some of the pieces. The broccoli was discarded.
Galley-Deck 4 - Vegetable Area
There were seams around the grease chutes where they penetrated the drip tray housing on the griddle and two flat grills. It appeared that the profile plates were missing. This was noted on the previous inspection. According to the staff, new profile plates had been received but they were too small.
Galley-Deck 4 - Coffee Station
There was a fly on one of the juice machine nozzles.
Potable Water-Reduced Pressure Assemblies
There were no test results for the reduced pressure assembly backflow prevention devices onboard. The records only included a date for the testing.
Potable Water-Backflow Preventer Inspections
Only the backflow prevention devices installed on galley equipment and cabin toilets had an inspection date in the records. There was no inspection information for the other non-testable backflow prevention devices installed onboard.
Potable Water-Cross-Connection Control Program
The list of backflow prevention devices did not indicate the total number of devices in some locations (e.g., cabin showers, balcony whirlpools and garbage room). The devices installed on the shower hoses in the medical center were not included on the list of devices.

In the technical compartment to the left of the cocktail station, the lower part of the pipes was soiled.

In the technical compartment for undercounter refrigerator R-12-05B, the insulation on the pipes was shriveled and soiled with a black material.

In the technical compartment for undercounter refrigerator R-12-07, the insulation on the pipes was shriveled and soiled with a black material.
Food Service General-Bar Multi-Flow Systems
There were no backflow prevention devices on the non-carbonated water lines to the multi-flow spray hoses and this was noted on the last inspection. The Bar Manager indicated that the vessel had been in contact with the company that installed the units, but the company said that they only install a non-return valve on these systems. The Bar Manager said that the vessel would install the backflow prevention devices.
Buffet-Lido Bread Station (Starboard)
There was a leaking container of soy sauce in the undercounter refrigerator next to the technical compartment R-14-16. This was corrected.
Galley-Dishwash
There was a chipped deck tile in front of the prewash spray. There was some recessed deck tile grout in the dirty dishwash area. This was corrected.

The gaps on the back of the turning handles for the tilting pans were soiled.

The horizontal profile strip on the lower bulkhead below the tilting pans was loose.

There were 5-6 small flies on the front exterior door panel of the reach-in refrigerator.

There were 2 small flies on the deckhead just outside the office door near the reach-in refrigerators.
Other-Laundry
During active cleaning of a variety of linen by more than six staff in the main laundry, the large, empty rolling bins labeled as clean were soiled with a variety of loose waste debris along the bottom interior surface and dirt debris. All the bins were set near the dryers with staff waiting to load the freshly dried clothes into the bins.
Other-Laundry
There was a heavy filth fly infestation in the main laundry during heavy cleaning and folding of linens. More than 20 flies were circulating around the area and inside bins of soiled laundry.
Potable Water-Cross-Connection Control Program
The inspection of non-testable backflow preventers was only recorded for the passenger cabin toilets. There was no inspection information recorded for any of the air-gaps or other non-testable backflow preventers installed throughout the ship, although staff stated that they were inspected in 2010.

The listing of backflow prevention devices onboard did not include the flexible shower hoses in the cabins for the Chief Engineer or the Master, and the two ward room showers in the medical center were not on the list either. The two chemical dispensing lockers for the housekeeping department on decks 8 and 10 had backflow preventers installed at the water connection. These preventers were not on the list. The pressure atmospheric vacuum breaker installed on the medical center dialysis hose connection (Watts 289) was listed as a Watts 9D.
Food Service General-Under-Counter Refrigerator Technical Compartment
The gray insulation material in many of the undercounter refrigerator technical compartments is shriveling. This was causing the sections to separate in some areas and the surfaces are difficult to clean. The food and beverage manager stated that the ship has submitted a warranty claim to repair this insulation.

In the technical compartment below the speed rail, the pipe insulation was soiled, including the multi-flow hoses.

In the technical compartment below the pasta cooker, there was a piece of metal in the back floor of the compartment. When the piece of metal was removed, the floor of the compartment below was soiled.

In the past three months, only one inspection was recorded at night for the housekeeping pantries. The integrated pest management plan indicates that night inspections should occur once a month.

There was no backflow prevention device for the potable water line to the multi-flow cabinets. There was a potable water line that went directly to the spray guns in the bar.

The drain line for the Oasis machine was in the handwash sink. It was moved to a utility sink.

There were several cooked items on the cooling logs, including chicken breast, ground pork, potatoes, sausage, and pasta that did not have the two hour temperatures recorded. According to the staff, the two hour temperatures were measured but not recorded. All of the food was below 41° within four hours of cooling. Also, according to the staff, pasta and potatoes were first cooled in an ice bath before they were placed in the blast chiller. However, the chicken, pork, and sausage were only cooled in the blast chiller.
Integrated Pest Management-
The records indicated that there was not an active night monitoring inspection in June for the Deck 3 & 4 food areas or the provisions areas. Per staff, there may have been confusion for AM and PM and the ship will switch to a 24 hour clock to avoid this in the future.

The cross-connection control program did not have a complete list of the guest toilet backflow prevention devices.

There were two pass through refrigeration units in the pastry area that were used as both for time control and temperature control. Per the time as a public health control plan, the units would be used for time control only during service times.
