Federal health inspectors from the Centers for Disease Control and Prevention inspect cruise ships twice a year, checking everything from kitchen equipment to dishwasher temperatures, and grade ships based on what they find. A score below 86 is considered failing. Cruise ships are also required to report all illness outbreaks affecting more than 3% of passengers or crew to the CDC. Inspections and outbreaks from 2010 to the present are shown below.

Medical-Food Handler Isolation
Medical staff did not document the last acute gastroenteritis (AGE) symptoms experienced by a crew member. A food handler on the voyage starting on 23 November reported to medical at 0855 on 29 November after experiencing AGE symptoms the evening prior. The 24-hour follow-up on 30 November indicated that the food handler experienced 3 additional diarrhea episodes after initially reporting to medical on 29 November, but the time of the last episode was not documented. The food handler was released from isolation at 0900 on 1 December and sent back to work. It could not be determined if the food handler was isolated for the required 48 hours.
Recreational Water Facilities-Forward Pool
The forward pool was in operation during the inspection and included interactive water features which were spraying and flowing. This interactive water facility was being operated as a swimming pool for the turnover rate and halogenation levels.
Buffet-Lido Starboard Back Service Line
The technical compartment to the right of the hot counter had two gaps around wiring coming through the side and back bulkhead.

The two small exhaust vents above both sides of the buffet counter were heavily soiled with an accumulation of dust.

The stainless steel plate on the far left end of the cold buffet countertop was not sealed completely to the countertop which created an difficult to clean seam around the plate.

A total of 5 movable food units were stored on the side of the pasta station. Three of the units had a hot plate surface with an undercounter storage cabinet, one had a large ceramic cooking surface, and the other had basic countertop with a sneeze shield and a neutral undercounter cabinet. According to staff these units had not been used in over a year.
Room Service-Flower Preparation
Several roses were stored in a reach-in refrigerator unit designated only for flower storage. According to staff the roses were cut and prepared in the soiled warewash area and then stored in the refrigerator for service.

Two moveable trolleys with a cold well and sneeze shield were stored at both waiter stations (one at each). According to staff these units had not been used in over a year.

The pipe connected to the intermediate vent of the double check valve backflow prevention device for combination oven #3 and #4 was sloped lower than at the discharge end by the floor drain which can allowed water to collect in the pipe. This set-up made it difficult to determine if there was a continuous leak from the intermediate vent indicating a possible failure.

The final rinse temperature of the in-use potwash machine was measured at 156°F by the inspector and 156°F and 157°F by crew. A 150°F/160°F/170°F maximum registering thermolabel was placed on a metal hotel pan and only changed color on the 150°F line. A 180°F maximum registering thermolabel was placed on the final rinse manifold and also did not change color, indicating the temperature was not reached.

The far left final rinse spray nozzle of the in-use mechanical dishwash machine was partially blocked creating an ineffective spray pattern.
Preparation Room-Dry Store Room
Approximately 3 to 4 small drain flies were observed around the stored bags of flour.

Several cases of liquor were stored in this room that did not have a hard, easily cleanable bulkhead or deckhead. Exposed wires were hanging from the deckhead and the bulkhead/deck juncture was not coved.

Only food provisions were recorded as being checked for pests. Staff stated they check all supplies (e.g. general supplies, engineering supplies, flowers, etc.) for pest but did not document inspections of nonfood items.

Potable Water-Production Records
On 30 November the evaporator was in use for 9 hours and no chlorine residual was documented. On 8 December the production record was missing due to a computer/printer communication error, and staff were unable to print the record to document the chlorine residual and pH values. Staff reported producing water for 7-8 hours this day. The electronic data logger was based on a real-time system and could only show the previous 24 hours.
Potable Water-Far Point Records
Some of the far point records did not verify the residual chlorine level to be at least 0.2 ppm for at least 16 hours each 24-hour period. On 25 October the chlorine residual level was missing for 15 hours and the chart recorder indicated 'chart recorder error.' No manual samples were recorded. On 20 October the residual chlorine level was less than 0.2 ppm for 11 hours. No notations were made and no manual samples were taken. On 5 October the chlorine residual level was missing for 11 hours. No notations were made and no manual samples were taken. On 16 September the chlorine residual level was missing for 9 hours and the chart recorder indicated 'chart recorder error.' No manual samples were recorded. The vessel was not in VSP's jurisdiction when these violations occurred.
Recreational Water Facilities-Records
On 8 December staff was manually recording the chlorine residual and pH values for the recreational water facilities every four hours since the computer was not working, but they were not testing every the whirlpools every hour as required.

The recreational water facility records did not contain notations to outline the action taken when the free halogen residual levels were outside the acceptable ranges during open hours. On 9 November the forward pool did not have chlorine residual recorded from 11:00-22:00 and no manual samples were recorded. A note indicated at 11:00 a chlorine probe was changed. On 19 November the chlorine residual of the mid pool was zero for 6 hours. On 16 November and 17 November the chlorine residual of the aft pool was less than 1.0 ppm for approximately 5 hours on both days. On 15 November and 16 November the chlorine residual of the mid whirlpool was less than 3.0 ppm for 3 to 4 hours on both days. The vessel was not in VSP's jurisdiction when these violations occurred.
Potable Water-Striping
The water lines were striped blue/green/blue before the chlorine injection point by the mineralizer and reverse osmosis plant.

The diaper station did not have a handwash sign to advise people to wash their hands after each diaper change.

A heavy accumulation of dust was observed on the bottom of the air filter compartment of unit #6.083. A similar finding was noted on the last inspection.
Recreational Water Facilities-Forward Pool
The forward pool was in operation during the inspection and included interactive water features which were spraying and flowing. This interactive water facility was being operated as a swimming pool for the turnover rate and halogenation levels. The features were shut off during the inspection, and staff noted they would consult the corporate office. Similar interactive features were noted on the mid pool, but it was closed during the time of inspection.

The forward pool had chlorine residual levels of 1.48 ppm and 1.47 ppm, as measured by the inspector's and ship's test kits. During the testing, the forward pool had interactive spray features in operation and users in the pool.

The air gap between the potable water supply and the technical water was less than 2-times the diameter of the pipe.

Most of the forward and aft far point halogen analyzer-chart recorder charts had a range of 0-10 ppm. Staff showed a work order which was placed on 12 January for charts with a 0-5 ppm range.

The air gaps for the mineralizer backwash drain lines were not included on the list of air gaps. This list was updated in January 2013. This was written on the last inspection report.
Recreational Water Facilities-Center Whirlpool
Upon entrance to the center whirlpool machine room, the halogen analyzer and chlorine dosing pumps' visual alarms were flashing, indicating an error. The analyzer indicated a free chlorine residual of 0.10 ppm. Upon arriving at the whirlpool a short while later, the attendants were closing the whirlpool until this system error could be corrected.
Recreational Water Facilities-Center Pool
Manual tests of the center pool indicated 0.88 ppm and 0.72 ppm by the staff member and 0.12 ppm and 0.26 ppm by the inspector. The pool was open at the time of the inspection. The halogen analyzer indicated 2.22 - 2.26 ppm. Staff determined that chlorine was supplied to the analyzer, but an abundant amount of air was supplied to the pool.
Potable Water-Distillate and Permeate Lines
After the mineralizers, the distillate lines from the evaporators and the permeate lines from the reverse osmosis system were not striped to the production chlorine injection point in accordance with ISO 14726 standards (blue/gray/blue).

The caps on the two lower potable water filling lines did not have their chains fastened to them.
Recreational Water Facilities-Whirlpool Safety Signs
The safety signs did not include 'no children in diapers or who are not toilet trained' and did not caution elderly persons and children using the facilities.

The safety sign for the water slide could not be located.
Buffet-Beverage Stations
The time control plans for the beverage stations did not identify the refrigeration units for the milk kept on time control. These refrigerators were not labeled 'time control'.
Galley-Entrance Corridors
There was no bulkhead/deck coving at the entrance corridors to the port and starboard pantries which service the aft grills. Food items and soiled dishes were transported through these corridors.

There was missing and pitted deck tile grout throughout this location.

There was mold growth in the food splash area on the back dispensing lip of the counter-mounted ice/water dispenser.

Water was leaking from a deckhead seam and pooling on the deck between the combination ovens and tilting pans. According to staff, a pipe in the deckhead was leaking, The leak was corrected.

There were broken deck tiles and missing deck tile grout inside the walk-in refrigerator and at the entrance to the walk-in freezer.

Approximately 30 individually packaged yogurt containers were holding at 45 - 55°F in the worker side undercounter refrigerator next to the self-service cold well. Also, there were approximately 30 individually wrapped packages of butter which were holding at 47°F. The refrigerator was on temperature control. Staff immediately discarded these products.

When the doors of the in-use combination ovens opened, excess condensation formed on the deckhead and bulkhead. Condensation was observed dripping onto the deck in front of the ovens and on the preparation table between the ovens. No food or clean items were impacted.

There was one live fruit fly at the back of the service line and one live fruit fly on the outside of the ice/water machine. The area was setting up for the lunch service at the time of the inspection.

Inside the in-use flight-type conveyor dishwash machine, the second to the right upper final rinse sanitizing spray nozzle was clogged, resulting in an ineffective spray pattern. Staff cleaned out the nozzle. The spray pattern was corrected.

There were at least five live fruit flies near the juice machine. The area was not in use at the time of the inspection.

The toilet room was not equipped with single-serve paper towels. This toilet room was used by crew members working in the nearby food areas.

There was a cart with plates near the entrance to the bar. It appeared these were for passenger self-service. The plates were not inverted or covered. Staff stated they handle these items.

Potable Water-Cross-Connection Control Program
The cross-connection control program for atmospheric vacuum breakers did not include a description for the plumbing system component for each device installed. The log only included the device model and a general description of the location. Also, the log did not include the air gaps for the mineralizer backwash drain lines or the air gap for technical water filling in the laundry room. In addition, there were no dates showing that the air gaps were periodically inspected.

The air gap for the technical water filling station was not at least twice the diameter of the potable water supply line.

The 'potable water filling' identification at both bunker stations was not clearly labeled with letters at least 13 millimeters high.
Recreational Water Facilities-Aft Swimming Pool/Water Slide
The water slide was not correctly identified as an activity pool and was not being halogenated consistently to at least 2.0 ppm free residual chlorine. The aft swimming pool and the water slide shared the same compensation tank and chlorine dosing, but the water slide did not empty directly into the swimming pool. The electronic data logger records for the aft swimming pool indicated that the free residual chlorine was below 2.0 ppm for several hours each day during operating hours over the last few months. The records also indicated that the pool was consistently opened with free residual chlorine levels below the required 2.0 ppm. When tested during the inspection, the free residual chlorine level in the water slide was measured at greater than 2.0 ppm.

Accountability of the passengers at each muster station was not taken during the passenger muster drill.
