Federal health inspectors from the Centers for Disease Control and Prevention inspect cruise ships twice a year, checking everything from kitchen equipment to dishwasher temperatures, and grade ships based on what they find. A score below 86 is considered failing. Cruise ships are also required to report all illness outbreaks affecting more than 3% of passengers or crew to the CDC. Inspections and outbreaks from 2010 to the present are shown below.

Potable Water-Far Point Halogen Alalyzer
A calibration check was conducted on the far point halogen analyzer. The analyzer indicated 1.0 ppm, but the crew member's test kit indicated 0.4 ppm and the inspectors test kit indicated 0.22 ppm.

The backflow prevention devices could not be located for the hair sink in the spa. Continuous pressure-type devices were installed.

There were broken tack welds on the cover to the ice bin compartment of the ice machine, causing a seam along the lower edge of the cover.

The vessel was conducting more than 20 microbiologic monitoring samples per month, but the locations each month did not change.

There were free halogen tests during bunkering for St. Kitts on December 23 but no E. coli test. There was an E. coli test for shore-side water in St. Maarten on December 24 but no free halogen bunkering tests. The bridge log book did not show water loading for St. Maarten. According to the Staff Captain, there was probably a mix up with the record and the E. coli test that was conducted was for St. Kitts on December 23rd.

The potable water hoses were not capped or coupled together .
Medical-
An asymptomatic cabin mate of a crew member with gastrointestinal illness did not have a 48 hour interview during the April 15-May 12 cruise.
Housekeeping-Outbreak Prevention and Response Plan
There was no disinfection frequency for the public toilet room, gym equipment, gangways or tenders when the cumulative proportion of gastrointestinal illness cases among passengers or crew reaches 2%.

A cardboard box was re-used to store bottled water.

There were openings around the deck penetrations pipes used for electrical lines.

In the buffet galley, the combination oven that was used for hot holding under time control and was not listed in the plan.

Safety
Serious crimes and injuries on cruise ships that make port in the U.S. are required to be reported to the Coast Guard. Smaller-scale crimes and thefts are reported to local police authorities (if reported at all). Incidents and inspections from 2010 to the present are shown below.

Serious crew member injury
During the replacement of ash grates in the lower incinerator ash chamber, the attending fitter who was standing with the upper part of the body inside the chamber, was injured when the lower chamber gate closed. The ships crew raised the alarm and he was subsequently freed by the efforts of the attending officers and crew. He was initially treated in the onboard hospital and then sent ashore to a local hospital, for an additional medical evaluation. The crewmember was released from the hospital few hours later, and rejoined the ship. He incurred brusing to the lower abdomen and anterior hip bones.

Personnel Casualties : 1 death
Ship's singer was found unresponsive in bed, bluish discoloration around the neck area and cyanotic. The medical team applied CPR. At 11:00J the crewmember was transferred to the ship's medical center. At 11:05J the crewmember was declared dead. Suspected cause of death: Cardio Pulmonary arrest. At about 16:00J, the body was taken in to custody by the police authorities in Darwin. The autopsy was performed in the morning of Feb 4, 2013. Ship's medical team suspects natural causes. Received notification from operator that cause of death was due to natural causes. The control of the report is under Australian control. 09Dec13 - Received Final Autopsy/Toxicology report conducted by the Royal Darwin Hospital Forensic Pathology in Australia. Report of cause of death concluded "undetermined" by Locum Forensic Pathologist. See reports in evidence section of this case. Based on the report of findings, this office could not conclude an accident occurred onboard the vessel. Case closed.
