Based on on records reviewed, interviews and observations for one of thirty sampled patients (Patient #1), the Hospital failed to: 1) Record Patient #1's presentation to the Hospital's Emergency Department (ED) on 1/1/18 in the corresponding ED Log and 2) Ensure the Hospital's Medical Staff Bylaws, Rules and Regulations identify the individuals qualified to conduct a medical screening examination in the Emergency Department.

Based on on records reviewed, interviews and observations for one of thirty sampled patients (Patient #1), the Hospital failed to record Patient #1's presentation to the Hospital's Emergency Department on 1/1/18 in the corresponding ED Log and the Medical Staff Bylaws did not define the individuals qualified to perform a medical screening examination (MSE).

Based on on records reviewed, interviews and observations for one of thirty sampled patients (Patient #1), the Hospital failed to record Patient #1's presentation to the Hospital's Emergency Department on 1/1/18 in the corresponding ED Log and the Medical Staff Bylaws did not define the individuals qualified to perform a medical screening examination (MSE). Findings include: The Hospital's ED log dated, January 2018, (MDS) dated [DATE], did not indicate Patient #1's visit to the ED on 1/1/18. The Surveyor observed, at 10:00 A.M. on 2/20/18 in the ED's electronic medical record, that Patient #1 arrived to the ED on 1/1/18. The ED Registration Supervisor was interviewed at 10:40 A.M. on 2/20/18. The ED Registration Supervisor said that Patient #1 came into the ED and then decided to leave so I canceled the reservation in the computer. The Director of Risk Management and the Director of the Emergency Department were interviewed together at 1:30 P.M. on 2/20/18. The Director of Risk Management said that because the ED registration clerks have access to cancel a reservation in the electronic system, the presentation of Patient #1 was not recorded in the Hospital's ED log. The Hospital's Medical Staff Bylaws, Rules and Regulations, dated 11/27/17, did not identify or specify the individuals qualified to conduct MSEs.

Notes

“Average time” refers to the median wait time (the midpoint of all patients' wait times). References to “doctor or medical practitioner” indicate a doctor, nurse practitioner or physician's assistant. CMS reports the CT scan quality measure as the percentage of patients who received a scan within 45 minutes. We have reversed that measure so that all measures follow a “lower is better” pattern.
