Issue:
Based on staff interview and record review, the facility failed to maintain the privacy and confidentiality for one patient (Patient A) when an employee was unable to locate a previous CD-rom record with Patient A's health information on it.Findings:An interview was conducted on 1/26/12 at 8:30 AM, with the facility's Privacy Health Information Officer (PHO). The PHO stated that on 11/23/11, she was notified that an anesthesia technician was unable to locate Patient A's previous ultrasound stored on the facility's CD-rom. Review of the information for Patient A that was possibly disclosed included:Patient nameMedical record test resultsThe PHO further stated that all staff were educated and in-serviced to prevent further occurrences.
