Issue:

Based on staff interview and record review, the facility failed to maintain the privacy and confidentiality of one patient (Patient A), when an employee inadvertently sent Patient A ' s insurance information to the wrong insurance company.Findings:An interview was conducted on 1/26/12 at 8:30 AM with the facility Privacy Health information Officer (PHO). The PHO stated that on 1/9/12, she was notified that an employee inadvertently sent Patient A ' s insurance information to the wrong insurance company. The PHO further stated that the facility became aware of the incident when the receiving insurance company called to let them know that Patient A was not a current member.Review of the following insurance information noted the following personal health information (PHI): Patient name Address Insurance informationDuring the interview with the PHO on 1/26/12 at 8:30 AM, she further stated that Patient A was notified of the incident. The PHO also stated that the employee was educated and disciplined.
