Based on staff interview and record review, the facility failed to maintain privacy and confidentiality for one patient (Patient A), when an employee inadvertently entered the wrong insurance information which resulted in the wrong insurance company receiving Patient A ' s information.An interview was conducted on 1/26/12 at 8:30 AM, with the facility ' s Privacy Information Officer (PHO). The PHO stated that on 1/9/12, she was notified that an employee inadvertently sent Patient A ' s insurance information to the wrong insurance company.Review of the information for Patient A sent to the wrong insurance company noted the following personal health information (PHI): Patient name Address Insurance informationDuring the interview conducted with the PHO on 1/26/12 at 8:30 AM, she further stated that Patient A was notified of the incident. The PHO also stated that the employee involved no longer works at the facility.
