Issue:
The Information Security Officer (ISO) previously submitted a ticket for a violation created by a contractor with the VA (PSETS0000118782). While they were investigating the initial incident, they discovered two other violations. On 04/23/15, they generated letters for four Veterans. However, the end result was two envelopes being mailed. Each envelope containing the letters of two Veterans.
Outcome:
05/15/15: The Incident Resolution Service Team has determined that the two Veterans whose information were disclosed will be sent letters offering credit protection services.
