Issue:
Veteran A received four pages of Veteran B's progress notes in the mail. The progress notes had Veteran B's name, diagnosis, medications, lab results, date of service. Veteran A talked with the Privacy Officer (PO) and mailed the information back to her.
Outcome:
06/11/15: The Incident Resolution Service Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
