Veteran A received a request for, and authorization to, release medical records and a health information form for Veteran B. The form had Veteran B's name, address, and full Social Security Number. Veteran A called in and mailed the form back to the Privacy Officer (PO).
Outcome:
06/11/15: The Incident Resolution Service Team has determined that Veteran B will be sent a letter offering credit protection services.
