Issue:

A Veteran called the National Call Center to check the status of his claim. While going over the status with the Veteran, It was noted by the Veteran, that we sent the letter to the wrong address. The Veterans address had been changed by mistake. The letter sent contained medical records which included his SSN, DOB, list of condition and medication. This was duplicate information we were returning to the veteran.

Outcome:

10/07/13: Veteran B will be sent a letter offering credit protection services.
