(Per operator) Veteran called in this morning stating that he had received a blood pressure cuff for another patient along with his own personal information. I returned the call to the Veteran and have not had a returned call yet.
Outcome:
05/16/14: The Incident Resolution Team has determined the patient whose name, address, and PHI was disclosed will be sent a HIPAA notification letter.
