Issue:

Based on interview and record review, the failed to prevent unauthorized access to a patient's (Patient A's) medical record, when an RN (Registered Nurse) gave another patient, Patient A's discharge instructions.Findings:On April 17, 2013, a record review for Patient A indicated on March 16, 2013, the patient was seen in the ED (Emergency Department). Upon discharge, the RN printed Patient A's discharge instructions two times. The RN gave one copy to Patient A, and another copy to another patient. The discharge instructions included, Patient A's name, date of birth, the medical record number, and account number.In an interview with the Executive Secretary, she stated, "On March 17, 2013, Patient B called and stated she was given incorrect discharge instructions. Patient A and Patient B were being discharged at the same time. Patient A's discharge instructions were printed two times. One copy was given to Patient A and one copy was given to Patient B."
