Issue:

Based on interview and record review, the facility failed to prevent unauthorized access to a patient's (Patient A's) PHI (Protected Heath Information) when Patient A's discharge instructions were given to another patient.Findings:A record review for Patient A was conducted April 17, 2013. The patient's record indicated, on March 4, 2013, Patient A was discharged from the hospital and her discharge instructions were handed to another patient. The discharge instructions included the patient's name, facility account number and Medical Record number.In April 17, 2013, during an interview with the Executive Secretary, she stated, "One March 4,2013, medical records personnel discovered the wrong signature on the discharge instructions. The forth floor director contacted Patient 2 who stated she had destroyed the discharge instructions."
