Issue:

Based on interview and record review, the facility failed to avert a patient (Patient A's) PHI (Protected Health Information) from being faxed to a number other than where it was intended.Findings:A review of Patient A's information revealed the facility staff faxed Patient A's medical record to an insurance company. Then when the the insurance company called to inform the facility they had lost their appeal, it was discovered the staff faxed the medical records to the wrong number.On April 3, 2013 at 9 a.m., the Executive Secretary stated, "Case Management was trying to fax discharge information to 'Medicare Appeals Office,' but was faxed to the wrong number."
