Issue:
Veteran A inadvertently received Veteran B's Exit Note as she was being discharged from the Dorn VAMC Women's Clinic. When Veteran A discovered the incident, she immediately contacted the Dorn VAMC Women's Clinic LPN to report the incident. Veteran A was asked to immediately turn over the documents to the Greenville CBOC.
Outcome:
05/24/13: Veteran B will be sent a letter offering credit protection services, as her full SSN and medical information were disclosed.
