Issue:

On 4/30/2013, a Dorn VAMC Womans Clinic LPN received a telephone call from a technician with a local Image Care office in Columbia, SC . The Technician informed the LPN that Veteran A presented to Image Care for a transvaginal/pelvic ultrasound that was ordered as a Fee Basis consult. The Veteran was mailed an authorization packet to her place of residence. The Veteran provided the authorization packet to the Technician at Image Care prior to her scheduled appointment. After reading the authorization packet, Amy noticed that another patients authorization was attached to the back of Veteran A packet. The Technician returned Veteran B's authorization packet back to the Dorn VAMC.

Outcome:

05/24/13: Veteran B will be sent a letter offering credit protection services, as her full SSN and medical information was sent to Veteran A.
