Issue:

Based on interview and document review, the facility failed, for one patient (Patient A), to ensure that Protected Health Information (PHI) were not disclosed to an entity not authorized to receive the information. As a result, Patient A's test results were mailed to Patient B, without authorization.Findings:On November 29, 2012, at 11:10 a.m., the facility's Director of Compliance, Law Department (DOC) was interviewed by phone. The DOC stated it was discovered on November 19, 2012 that a volunteer put one page of Patient A's test report, which contained the patient's name and address, in Patient B's envelope and mailed it to Patient B. Patient B called the facility on November 19, 2012 about the mistake. Patient B destroyed Patient A's test report. Patient A was notified by letter with the unauthorized disclosure of an information privacy breach on November 27, 2012. On November 29, 2012, a review of facility documents included:a. A letter of Patient A's mammography examination result. The PHI included the Patient's name, address, medical record number, and the final test result. b. The letter addressed to Patient A, dated November 27, 2012, indicated the facility notified Patient A of the privacy breach.
