Based on interview and record review, the facility failed to prevent unauthorized access to a patient's PHI (Protected Health Information ) when a outside billing company received was sent Patient A's PHI.Findings:On February 26, 2013, MD 1, dictated an operative note using MD 2's dictation number, which included Patient 1's name, date of birth, and medical record number. The document was then sent to an outside company for billing. In an interview with the facility's CO (Compliance Officer), she stated, "A medical report was dictated under a different patient's name, date of birth, and medical record number then was sent to an outside facility for billing."
