Issue:

Based on interview and record review, the facility failed to prevent unauthorized access to a patient's medical information when a patient's (Patient A) prescription was given to another patient.Findings:On February 5, 2013, during a physician appointment, Patient A's prescription for antibiotics was given to Patient B. The prescription included the patient's name, date of birth, name of medication and the patient's medical record number.On April 5, 2013, at 9:50 a.m., an interview was conducted with the facility's CO (Compliance Officer). She stated, "Patient A's prescription was given to another patient (Patient B). The Patient B presented the prescription to the pharmacy. The prescription was returned to the facility."
