Issue:
Based on interview and record review, the hospital failed to ensure that Patient 1's medical information was kept confidential from anyone not authorized access.Findings:An interview with the Director of Risk Management (DRM) was conducted on 5/15/12, at 2:50 P.M. The DRM stated that a CD (compact disc) of Patient 1's radiology procedure and result was given to the wrong patient. The DRM stated that Patient 1's CD was not viewed because it was discovered by the receiver that the name on the CD was not the patient. However, the CD label had Patient 1's name, identification number, the procedure and the date it was done. The DRM acknowledged that the hospital staff should have checked the CD to ensure that it was being given to the right patient.
