Veteran A received a notification letter. Attached to his notification letter was a letter addressed to Veteran B's Doctor for release of medical records. Veteran B's letter contained his DOB, SSN, File number, medical conditions, first, and last name. Veteran A returned letter to the Regional Office of Jurisdiction (ROJ).
Outcome:
11/24/14: The Incident Resolution Service Team has determined that Veteran B will be sent a letter offering credit protection services.
