Issue:

On 05/08/15 at 4:22 PM, the Privacy Officer (PO) read an email from the Assistant Pharmacy Chief that she sent on 05/08/15 at 4:02 PM. Veteran A called that said that he received a UPS package with his name on the outside of the box, however, when he opened up the box there was insulin for Veteran B and that it was mailed on 05/06/15. Veteran A was looking for his hydrocodone which was mailed on 05/07/15. The Assistant Pharmacy Chief contacted the mailroom and they are tracking the hydrocodone. The records show that it was delivered on 05/08/15 but there is not a signature. The mailroom said they would contact UPS and find out where it is. The Assistant Pharmacy Chief is shipping more insulin to Veteran B. The mailroom does not think Veteran B could have received anything from UPS (would not have the hydrocodone) because Veteran B is not in the UPS database so we have never mailed anything to him. This involves Veteran Bs full name, DOB, SSN, address, actual medication (insulin) and medication list.

Outcome:

05/11/15: The Incident Resolution Service Team has determined that Veteran B will be sent a letter offering credit protection services.
