Issue:
On 05/08/15 at 4:19 PM, the Privacy Officer (PO) read an email from the Pharmacy Chief that she sent on 05/07/15 at 1:09 PM that hydrocodone intended for Veteran A was sent to Veteran B. The incorrect mailing label was placed on the package by the mailroom. This involves Veteran As full name, DOB, SSN, address, actual medication (hydrocodone) and medication list.
Outcome:
05/11/15: The Incident Resolution Service Team has determined that Veteran A will be sent a letter offering credit protection services.
