Federal health inspectors from the Centers for Disease Control and Prevention inspect cruise ships twice a year, checking everything from kitchen equipment to dishwasher temperatures, and grade ships based on what they find. A score below 86 is considered failing. Cruise ships are also required to report all illness outbreaks affecting more than 3% of passengers or crew to the CDC. Inspections and outbreaks from 2010 to the present are shown below.

Medical-Crew Member Acute Gastroenteritis (AGE) Reporting
On the cruise dated 8-15 June, a spa employee had an onset of AGE symptoms on 13 June at 0800 but did not report to the medical center until 13 June at 1834. The employee was a reportable AGE case. The work records were checked and the employee worked the morning of 13 June. On the cruise dated 29 June to 6 July, a team leader from the deck department had an onset of AGE symptoms on 1 July at 2000 but did not report to the medical center until 2 July at 1600. The team leader was a reportable AGE case. The work records were checked and the team leader worked several hours on 2 July. This case had been referred for review by the ship's management.

The bather load was not listed on the safety sign.
Potable Water-Distribution Chlorination Station Back-up Pump
The back-up pump installed at the chlorination station did not have an automatic switchover feature and could only be switched over manually. Staff was aware of this issue and a were waiting for spare parts to repair the pump system.
Galley-Main Galley - Center Hot Galley
There were some cracked and broken deck tiles near the three deep fryers and the two range tops in the center hot galley section. Additionally, there were several replacement deck areas where it was difficult to verify as clean due to the dark cement residue on top of the tile surfaces. This was especially true in areas beneath equipment, counters, racks and grills.

There was food debris, dust debris and some liquid residue along most of the surface of the long service counter overshelf where previously cleaned plate covers were stored. Staff removed the plate covers for rewashing and cleaned and sanitized the overshelf.

One live fruit fly was noted on the side surface of the service counter overshelf in this pantry, and another was observed on the deckhead above the clean storage rack and opposite the blast chiller.
Food Service General-Galleys and Pantries - Deckhead
In nearly all areas of the galleys and pantries the deckhead attached stainless steel plates were partly detached from the deckhead or simply missing screws meant to fasten them tightly to the deckhead. In some cases there was a corner of the panel hanging down and creating a gap. Some of the deckhead access hatches had a similar condition with a missing or non-working fastener. In a few cases, such as in the main galley potwash, there was water dripping from the open panel or the screw hole for that panel to surfaces below.
Galley-Main Galley - Pot Wash
During active use of the flight-type conveyor dishwash machine the mounted rinse water temperature gauge displayed as 156 °F, which was confirmed by measuring the rinse tank manually using the inspector's and ship's thermometers. Later the gauge reading displayed 150 °F, but the manual reading from the rinse tank was 162 °F. The manufacturer's data plate specified a minimum 160 °F rinse temperature.

There was a significant deckhead water leak throughout the clean pot storage room, which had already been identified by the ship's staff, who had removed the clean equipment as they were trying to locate and control the leak.

During active use of the port side flight-type conveyor dishwash machine the mounted rinse tank thermometer gauge displayed 150 °F. A manual temperature of the rinse water tank registered 155 °F. The machine data plate specified a minimum rinse water temperature of 160 °F.

There was a heavy and consistent water leak from under the in-use flight-type conveyor dishwash machine that was pooling water on the deck at a rapid rate. With the internal curtains removed it was noted that the side panel filling line was turned in an upward direction and was spraying water into the side wall of the machine. Staff repaired this immediately and the leaking water stopped.
Dining Room-Truffles Dining Room - Aft/Port Waiter Station
The soiled storage counter was filled with various soiled china and utensils, but on an open shelf below was stored a large bin of individual sugar packets. Although none of the sugar packets were soiled they were not protected so staff moved them to a clean storage area.

Several soiled items, including a bin of soiled liquid were stored on the soiled counter at waiter station #2. On an open shelf directly below the counter was stored a tray of 30 previously cleaned small ramekin butter dishes which were all inverted on a towel covered tray.

One filth fly was observed on the serving counter near the cold well.

A live filth fly was observed at the buffet clean utensil counter. A live fruit fly was observed on the deckhead directly above the center hot buffet line.

There was a consistent brown staining of grease residue throughout the pin hole type deckhead tiles in the crew mess buffet. The deckhead mounted lights over the crew mess beverage station were heavily soiled with dust debris and drink splash.

There was an accumulation of dust debris on the decorative wood piece attached to the deckhead directly above the beverage station. The adjustable deckhead light fixtures were heavily soiled with dust debris beyond one day's accumulation.

The grill was in active use and 3 filth flies were circulating in the area during preparation.
Galley-Lido Galley - Dishwash
During active use of the flight-type conveyor dishwash machine there was a deckhead water leak which dripped down to one of the clean tables beyond the clean end of the machine. Staff stated this was due to previously opening the dishwash machine.

The previously cleaned and not in use front galley had a visible oil residue splashed along the deckhead over the entire front service counter.

Recreational Water Facilities-Analyzer Chart Records
Sea-to-sea mode was not written consistently on the chart records for the three pools. At least 20 dates between the three pools going back to September 2013 were found where the pools were in sea-to-sea mode but no notation was made on the analyzer chart record. Several more records showed the three pools opening in recirculation mode and switching to sea-to-sea mode later in the day again with no notation made on the analyzer chart record. No calibration record was made on the analyzer chart recorder for the aft pool on 30 January 2014 and 31 December 2013. No calibration record was made on the analyzer chart recorder for the forward whirlpool 18 January 2014.

Two salamander broilers were heavily soiled with old grease in the technical space with the electronic control wiring, control boxes and broiler shelf lift mechanism. These broilers were cleaned while the inspection team was in the area.

Rust residue was found in the water recirculating bath in the ice maker unit of the ice machine adjacent to the pot wash area.

Frozen leaks were noted from the deckhead seams. Ice had accumulated on the deck at several places below the leaks. These leaks were from condensate from the recent dry dock where the entire freezer deckhead had been opened up. No food was impacted by these leaks.

A black substance that appeared to be mold was observed on the lower edge seam of the ice storage bin deflector plate in the ice machine. The ice maker unit of this pantry ice machine was completely clean. The machine was taken out of service and cleaned.

Cooling records for three different meat products indicated that the cooling procedures were not monitored correctly. The records showed that temperatures taken for the final 6 hour time control point were taken after the 6 hour mark had passed for the following food items: pork ribs (cooked on 6 November), short ribs (7 November) and lamb shank (7 November). The temperatures taken for each product were below the required 41°F but it was unknown if the products reached this temperature in the required 6 hours. The temperatures of these food products were measured betwen 6 hours and 15 minutes and 6 hours and 30 minutes. These records were signed off by supervisory staff.

Light intensity could not be raised to the required 110 lux of light behind and around various pieces of counter and deck mounted equipment. Some of the equipment included: combination ovens, hood-type potwash machines, beverage station equipment, and deck mounted ice machines. A plan to address lighting requirements was observed.
Recreational Water Facilities-Fecal/Vomit Accident Response
For the vomit accident which occured in September 2012, the record showed the cleaning portion was done with 200 ppm chlorine, which is not an effective sanitizer. Further review of the written plan includes direction to clean and disinfect with a 200 ppm chlorine solution.
Recreational Water Facilities-All RWFs
The bather loads and turnover rates for all RWFs were made from theoretical pump calculations. No flow meters were installed to provide actual circulation flow rates. Proof of an order for this equipment was provided from August, with follow-up in late October that the items were ready to ship.

The single posted health and safety sign at the aft pool was 26 feet away from the position opposite of where guests would attempt to read the sign, but the lettering sizes vary and only 3 of the messages are readable from that distance. Two additional signs of the same size should be posted at each side entry to the pool should be posted.
Recreational Water Facilities-Pools and Whirlpool Spas
There were several days over the last two months where the pH values recorded on the analyzer charts registerd above 7.8. For the spa whirlpool, there were 8 or more days the pH was between 8.12 and 8.4. For the aft, Serenity, and whirlpool spas, nearly all the charts showed a pH over 7.8, with several days reaching 11.2. The pH for the mid pool on recirculation days averaged 8.4, the forward pool averaged 8.12, and for several days the crew pool pH was between 8.12 and 8.4. There was no indication of an alarm, and the pools and whirlpool spas were not closed due to high pH. Also, it did not appear that there was a calibration check made for each day the RWFs were used in recirculation mode.

It was not clear that the lack of an immediate contact was to be recorded and where it was to be recorded in the electronic gastrointestinal illness database. Some staff did note in a comment box if there was no contact, but it was not clear in cases where a cabin mate was identified that there was no other immediate contact involved with the crew GI case.

The turnover rates for the combined RWF's could not be determined, but it was clear that the swimming pool would not meet the required turnover rate and disinfection requirements for the children's pool.

Safety signs and additional safety signs for whirlpools did not contain all the required information in the 2011 VSP Operations Manual. According to the staff, new signs have been ordered and will be installed as the signs are available.

Housekeeping-Showerhead Disinfection
Showerhead disinfection logs listed HB-Quat as the chemical used to disinfect showerheads and not a halogen-based chemical disinfectant. According to the staff, showerheads are first disinfected in HB-Quat, then disinfected using chlorine.

Two suction drain covers loose and/or missing fasteners. In addition, two suction drain covers not oriented as directed on the raised lettering of the covers. The drain covers were installed upside down.

The hot display cabinet in the omelet/pizzeria outlet was labeled time control and was listed on the time control plan, but the foods are for food display only and are not sold or used for consumption. The time control label was removed from the unit and the unit was removed from the time control plan.

A worker in charge of this area put a thin piece of cardboard on top of these food items when they noticed the leak but continued to store the food items directly under this leak and under the evaporative cooler that was leaking. There were also several boxes of wine that had water leaking onto them from the deckhead panels above.
Provisions-Beer and Soda Room
There was water dripping from the deckhead and the evaporative cooler onto several boxes of dried bowl noddle soup containers. A worker in charge of this area put a thin piece of cardboard on top of these food items when they noticed the leak but continued to store the food items directly under this leak and under the evaporative cooler that was leaking. There were also several boxes of wine that had water leaking onto them from the deckhead panels above. This same individual in charge stored 6 kegs of beer on the deck. When the inspector asked this worker why they had been placed there, they replied that it was only temporary storage; however several cases of soda were also stored on top of some of the kegs. Finally, one of the workers in charge of this area, had placed a cardboard box of items that were supposed to be taken to the bar including two small plastic containers of m ilk, and on can of unopened whipped cream. These products were over 55 degrees Fahrenheit when tested by the inspector. According to the staff in charge, these items were placed here earlier in the morning. The items were discarded.

There were 6 kegs of beer stored on the deck. When the inspector asked this worker why they had been placed there, they replied that it was only temporary storage; however several cases of soda were also stored on top of some of the kegs. In addition, there was black mold and mildew along the entire horizontal deckhead seam that ran the length of this rooms and several vertical seams. There was water collected in several areas of the deckhead, including in some of the light covers. Food items were stored directly under all these areas.

Ensure that for each restaurant the menu's used have notations at each animal food which is served by order or by recipe in a raw, undercooked or processed way to advise consumers of the elevated risk of consuming such foods. In food outlets either a sign, placard, menu or other written method at the location should be used.

The ship's staff were not equipped with sensitive tip food thermometers for measuring temperatures of thin foods.

There was a mixture of the required elements on all the time as a public health control plans in the various galleys and food outlets throughout the ship. Galley plans contained a mixture of galley specific equipment on time control and dining room or buffet area equipment on time control. Restaurant pantries on decks 2 and 3 did not have the bain marie (soup station) or attached undercounter hot cabinets labeled as time control. Most plans provided only the discard times for foods and no set-up times. In the crew galley there were potentially hazardous foods on ice baths at counters on time control, but that method was not listed in the time control plan. Additionally, there were pieces of cooking equipment with various foods inside and time control labels attached, but that equipment was not listed on the time control plan (steam kettles, range tops). Those equipment pieces were not labeled as time control units.None of the bain marie's in galleys, pantries, buffets, or room service were labeled time control and none of the attached warming cabinets below them were labeled time control either.
Dining Room-Port and Starboard Pantries Decks 2 and 3
The starboard deck 2 pantry had power cables for the coffee and juice machines draped on the counter making cleaning difficult. Power cables were on the counter top behind the bulk milk dispensers and coffee machines in the deck 2 port pantry, and the same behind the deck 3 port side juice dispenser.

During active use of the flight-type conveyor dishwasher two of the upper arm auxiliary rinse spray nozzles were fully clogged and two others were spraying only weakly. Additionally, the rinse tank temperature was measured at 163 °F, but the mounted digital thermometer displayed a rinse tank temperature of 176 °F.

The in-use flight-type conveyor dishwasher had a heavily soiled upper steel panel which was postioned just after the final sanitizing rinse. This panel soil was beyond a day's accumulation.

The 7-day discard labels for the rigatoni salads stored in the walk-in refrigerator were labeled with production date of 9 October and discard on 15 October, but in the cooling log entries for the rigatoni pasta and cooked vegetable ingredients for this salad they were actually cooked on 8 October and cooling ended by 7 pm that evening. Final preparation of the other ingredients to the salad was on 9 October.

Water was dripping from a heavily mold soiled plastic speaker cover in the deckhead over the aft food preparation counter. The counter was clean and not in use at the time of inspection.

The deckhead attached light fixtures were loose from the deckhead as were many deckhead panels over the buffet line and adjacent beverage station, making the area difficult to clean.

The sanitizing solution at this in-use outlet was very cloudy and the free chlorine concentration was below the 10 ppm minimum on the inspector's test strip. The water was discarded immediately.
Food Service General-Pot Wash Machines
At least three pot wash machines were in significant disrepair in the main galley and room service pantry. Staff were attempting repairs on these and the conveyor machines throughout the inspection and there was a significant accumulation of soiled pots and equipment especially in the main galley.
Medical-Crew Gastrointestinal Illness Reporting
The review of the gastrointestinal illness (GI) logs for the last 5 cruises revealed that there was a barista who experienced GI symptoms and continued to work after those symptoms began. Specifically, she reported to experience 3 vomit episodes and headaches with an onset of symptoms on 6 September 2011 at 1329 but reported to the medical center on 7 September 2011 at 1400. She worked her shift on 6 September 2011 but did not work on 7 September 2011. Her whereabouts from the time she sighed off to the time she reported to the medical center were unknown.
Housekeeping-Restrooms
There were no signs advising users of the toilet facilities to use hand towel, paper towel, or tissue to open the door. However, there were paper tissue dispensers on the bulkhead next to the exit doors.
Recreational Water Facilities-Automated Free Halogen Residual and pH Testing
There were no chart recorders or electronic data loggers with security features that record pH and halogen measurements for each individual recreational water facilities.

On 28 August 2011 a potable water sample tested positive for coliforms but there was no documented result for E. Coli testing. The staff stated that they did the check for the presence of E. Coli but the result was not recorded. It was unclear if the test was positive or negative for the presence of E. Coli.

DO NOT USE (whirlpool/spa)-Filter Backwashing Records
The log form used for whirlpool spa monitoring and maintenance included a statement in the comment box: 'all sand filters backwashed until clear water was visible on the sand filter'. In discussion with the engineer responsible for this maintenance, it was determined that the filter backwash was an automatic process and he did not view the water moving through the filter to see if the water was clear or not at the end of each backwash cycle. The form had this statement each day, so it appeared the wording was simply part of the forms contents rather than a record of what was actually done.

SSP rcvd a call fm D7 that C/S Carnival Legend was experiencing propulsion issues due to intermittent operation of the port and starboard Azipods propulsion systems. The vessel encountered the issue after making a port call in Rotan, Honduras and had to cancel a port call at the Grand Cayman islands. It was due into Tampa at 0400 local on 17MAR13.

A CG-2692 was submitted to Sector Miami reporting that a 41 YOF passenger on the CARNIVAL LEGEND had sustained an injury when the passenger was ascending steps leading to the dance floor when the heel of her left shoe caught in the edge of the first step. When trying to dislodge her show from the metal step, the passenger lost her balance and struck her left mid- shin against the next step. Ship's doctor diagnosed the passenger's injuries as fractures on her left shin bone. CG-2692 indicated that the injured passenger was under the influence of alcohol at the time of the incident.
