Federal health inspectors from the Centers for Disease Control and Prevention inspect cruise ships twice a year, checking everything from kitchen equipment to dishwasher temperatures, and grade ships based on what they find. A score below 86 is considered failing. Cruise ships are also required to report all illness outbreaks affecting more than 3% of passengers or crew to the CDC. Inspections and outbreaks from 2010 to the present are shown below.

Staff stated that temperatures were acceptable several hours ago when they first checked the machine. The final rinse gauge was measuring 180°F and spray patterns were found acceptable. The machine was stopped for repairs.

The wash tank temperature of the in-use rack-type glasswasher was 166°F. However, the wash temperature gauge was measuring 141°F. Staff stated they knew that the gauge was faulty. The gauge was repaired during the inspection.

A container of anchovies and a container of hummus were placed on time control in an undercounter refrigerator and both containers had seven day discard labels attached to the containers. The flexible shower connection in the toilet room had no backflow prevention device attached. Staff installed a backflow preventer to the shower hose connection during the inspection.
Recreational Water Facilities-Forward Pool, Slide and Child Pool
The recirculation, filtration, and halogen/pH dosing system is combined for the forward pool, slide pool, and child pool (located near Camp Carnival). The turnover rate for these three RWF's is 2.5 hours, and not the required 0.5 hours for a children's pool.

Food Service General-
Outstanding compliance with the USPHS Operations Manual Guidelines was noted in the food operations aboard the Carnival Liberty today. The critical areas of temperature / time control for potentially hazardous foods, the extensive dishwashing and potwashing operations, and the overall level of food facility cleanliness was notable. The food staff and crew were very well trained in important public health details and their execution of their program to safeguard the health of the passengers and fellow crew members was exemplary.
Potable Water-Production Records
Since the last inspection, a few written records indicated the free chlorine residual of at least 2.0 ppm was not documented within 30 minutes of the start of production. Staff stated the start time written on the forms indicated when the evaporators were turned on and that it could take some time for adjustment before the actual start of production.
Housekeeping-Serenity Area Variance
The ship received an approved variance to restrict children in the Serenity Area and to be exempt from words and phrases on the Serenity Area safety signs pertaining to children and those who are not toilet trained. During the inspection, the variance could not be located. The variance and signed approval letter are required to be kept onboard and available for review.
Recreational Water Facilities-Whirlpool Shock Treatment
Shock bromination information was very different from what was written on the chart records to what was entered on the computer printed records. For example, all of the chart records indicated a written start concentration of approximately 10.1 ppm and a written stop concentration of approximately 10.5 - 10.8 ppm. The computer printed records indicated a start concentration of 50 ppm and a stop concentration of 10+ ppm. Staff stated their water test kits measured up to 10 ppm bromine and any measurement above 10 ppm woud result in an error message on the test kit. There was no way to determine how 50 ppm bromine was obtained.

The time control plan for hot and cold food stated that only cold potentially hazardous foods were on time control. The staff stated that foods like hot pizza were also on time control.

The safety sign had all required information, but its location and 45 degree angle facing the deck meant that users had to enter the pool in order to read the sign.

The previous inspection disclosed that the sample points for monitoring both whirlpools were taken from the compensation tanks. It was unclear from the current inspection and conversation with the person in charge if this item was corrected. In the machinery space the inspector asked about a sample point that appeared to be in the correct location and the crew member did not know what it was for. Further documentation indicated that this point was the monitoring sample point and that its placement was correct.
Recreational Water Facilities-Pool Outside Children's Center
There was no separate monitor for this pool. The pool was on a combined system with the slide and the slide pool. This was noted in the previous inspection.

Water was dripping from the deckhead onto boxes of kiwi and other fruit stored on the center shelves closest to the door. A significant amount of water was pooled on the top of the kiwi box and leaking into the box. The water was coming from the seam in the deckhead above the center storage shelves.

The scrapping table and soiled landing were stacked full with soiled items, and there were four additional deck stands 4 to 5 feet in length stacked with soiled items 3 feet high. Ultimately, soiled pots and pans were being cleaned manually in the 3-compartment sink as well as sent to working potwash facilities on other decks.

Also, the light intensity was less than 220 lux at the forward starboard and port beverage stations during self-service. According to the staff, a light plan was in place for several of these areas.

Housekeeping-Outbreak Prevention and Response Plan (OPRP)
The OPRP did not have a way to notify passengers and crew members of an existing outbreak as well as informing passengers embarking the vessel following an outbreak voyage. The plan had letters in place that were to be distributed to the passengers and crew for both scenarios, however, the letters that were observed did not indicate that an outbreak was either occurring or had occurred, rather the letters were fact sheets on Norovirus.

At 1130, there were multiple raw eggs from different cases and different locations within the walk-in cooler with temperatures ranging from 61°F to 65°F. In addition, there were multiple individual non-UHT yogurt containers with temperatures ranging from 56°F to 57°F. At 12:00, while the crew was still provisioning and with the door of the cooler open, the temperatures of these food items were rechecked. Some eggs of different cases had temperatures ranging from 50°F to 61°F and some yogurts from different cases had temperatures of 42°F to 47°F. The room temperature was measured at 48°F. All temperatures were measured by the VSP inspectors and the staff personnel. The staff attributed these temperatures to the open door of the cooler during provisioning. The staff personnel also stated that the pallets with these products were taken straight from the refrigerated containers in the shore side to the ship's walk-in cooler, having these pallets out of cooler temperatures for a maximum of 20 minutes. The staff personnel also presented a temperature log to the VSP inspectors showing the temperatures of these items upon receiving. All receiving temperatures ranged from 38°F to 40°F according to the receiving log. However, because there was no time recorded in the logs as the time when the temperatures were taken, there was no assurance that these potentially hazardous foods were held at temperatures above 41°F for less than four hours.

Medical-Crew with GI Reporting to Medical
On 11 April 2011, a Camp Carnival Staff member, who was classified as a "food worker" had an onset of signs and symptoms of acute gastroenteritis (AGE) starting at 0400. The staff member reported to medical and was isolated starting on 11 April 2011 at 1426. According to the Camp Carnival manager, the staff member did not work from the period of onset to the end of isolation 48-hours past the last symptom. On 27 March 2011, a female team waitress with signs and symptoms of AGE was isolated starting at 1727. According to the medical record, the patient reported to medical earlier in the morning, with a chief complaint of "not feeling well." When the patient reported to medical in the afternoon, the patient indicated that she had signs and symptoms of AGE that began on 26 March 2011, starting at 1300. According to the Hotel Manager, the waitress did not work from the period of onset of signs and symptoms until the end of isolation 48-hours past the last symptom. On 29 March 2011, a galley steward with signs and symptoms of AGE (6 loose stools and stomach cramps) was isolated starting at 1847. The steward 's onset time for signs and symptoms was 0700 on 29 March 2011. According to the Hotel Director, the staff member did not work from the period of onset until the end of isolation 48-hours past the last symptom.

On 15JUN13 at 1430, a passenger broke his ankle while retrieving a basketball on the C/S CARNIVAL LIBERTY while the vessel was moored at Terminal 9 in the Port of Miami. The injured passenger was playing basketball in the basketball court on Sports Deck (Deck #12 amidships starboard side) and accidentally slipped on a wet deck while retrieving a ball from against the partition. The passenger's right foot got caught between the floor and the partition, causing a fracture in the passenger's right ankle. Injured passenger was disembarked in Miami for further medical treatment ashore at Jefferson Memorial Hospital.

On 8 Jul 2011, the Coast Guard was notified that on 7 July 2011, a female passenger of the CARNIVAL LIBERTY was walking on the Lido Deck and slipped on the greasy wooden deck outside of the starboard hot galley door. She fell on the deck landing on her right hand. An X-ray was taken by the ship's duty nurse and it revealed a fractured right wrist. Pax will seek further medical treatment when the vessel moors in Miami.

CARNIVAL LIBERTY (O.N. 9278181): Sector Miami rcvd a report from cruise ship requesting a MedEvac. female developed abdominal pain evening of 05/27/2011. Pain became gradually more severe over the course of 05/28/2011 but she did not present to the medical center until 1912LT, 3 hours after sailing from Miami. Following an assessment a diagnosis was made . At about 2157LT, subject guest was airlifted thru USCG Helicopter along with Duty Nurse to Jackson Memorial Hospital for further medical evaluation. Downgraded to Data Collection due to the incident involves injuries resulting from a pre-existing medical condition and not from a marine operation.

CARNIVAL LIBERTY (O.N. 9278181): A male (US Citizen) found unresponsive in cabin. Passenger pronounced dead while onboard. Downgraded to Data Collection level of investigation after report indicated death from natural causes. No drug or alcohol tests were conducted on the CARNIVAL LIBERTY's crew as no inaction or actions made by the crew contributed to the passenger's death.

The Medical Staff on Carnival Liberty rcvd a call via internal 911 system that a XX YOA female passenger in cabin 6285 was not breathing. The ship's Physician and nurse responded and found the passenger in bed, unresponsive and cold to the touch. CPR was immediately started but to no avail. Dr. pronounced her dead. PAX had surgery in April, scheduled for stent but it was deemed risky.

Material Failure (Vessels)
Carnival Liberty's #5 Diesel Generator was stopped due to an increase of exhaust gas on both banks and overload on both TT/CC. Diesel Generator #1was shut down by automation due to oil mist in the crankcase, cooling water was leaking from the turbocharger and cylinder heads on both banks. Diesel Generator #5 was repaired and back in service on 6 November. Diesel Generator #1 was repaired and back in operation 0n 11 November.
