Federal health inspectors from the Centers for Disease Control and Prevention inspect cruise ships twice a year, checking everything from kitchen equipment to dishwasher temperatures, and grade ships based on what they find. A score below 86 is considered failing. Cruise ships are also required to report all illness outbreaks affecting more than 3% of passengers or crew to the CDC. Inspections and outbreaks from 2010 to the present are shown below.

Medical-Acute Gastroenteritis (AGE) Case Definition
Medical staff did not fully understand the AGE case definition. On 27 July, a passenger reported one vomit episode and was deemed a reportable case because the one vomit was above normal to that individual. Staff acknowledged that they understood that above normal vomits also met the definition of an AGE reportable case.

Potable Water-Dialysis Machines
The ship did not have backflow protection for dialysis machines. Staff stated that passengers could bring their own dialysis machines. Because the machines need running water to function, passengers connected the machines to potable water connections in their cabins. Staff stated that passengers could make connections to the sink faucet or the shower hose in the bathroom of the cabins. The faucet connection did not have backflow prevention and the atmospheric vacuum breaker on the shower hose connection did not offer adequate backflow prevention.
Medical-Pantry
In the technical compartment of the undercounter refrigerator, there were gaps where pipes penetrated the back bulkhead. There was no back panel in the technical compartment.
Potable Water-Microbiologic Monitoring
Both color comparators for analyzing results of the potable water testing method (Colilert) for microbiological monitoring expired in July. There was a requisition from June for a new comparator.

There was no air gap at the air relief vent of the reduced pressure assembly. An inverted plastic bottle with an open bottom was surrounding the vent.

To the left of the corridor entrance, along the passageway from the dirty ware drop off near door FSDA4.18 a constant drip was observed from the deckhead seam. Crew thought it was from condensate dripping from the air conditioning ducts above the deckhead. No food was being impacted. This problem was not noted in other areas of this galley.
Provisions-Frozen Fish Storage
An icicle had formed from water leaking from two deckhead panel seams near the back bulkhead in the first row of storage shelves. Another frozen leak was observed from the deckhead/bulkhead seam in the same area. Ice was found on the shelf, not on the cartons of food stored in the area.

A slight leak was noted above the aft walking area between shelves of beverages stored in carton. The deck was wet from this leak. No food was impacted.

Also, the metal profile strip along the counter bulkhead juncture was not attached properly to the bulkhead creating an open gap that was soiled with food residue.

Also, a small steady stream of water ran continuously from the edge of the exhaust hood to the deck along the right bulkhead at the clean landing. No clean equipment was affected.

Also, the 'Beverages' sign at the starboard beverage station located above the condiment rack had a heavy layer of dust buildup.

There were open gaps between the deckhead slat panels above both ice cream stations and waiter stations which were open to the open plenum space. The sides of the these deckhead panels were also heavily soiled with dust.

Recreational Water Facilities-Forward Pool
The forward pool was open and in use with both children and adults at the time of the manual testing for water chemistry. The ship's test kit registered free chlorine concentrations of 1.52, 1.28, and 1.28 ppm in three different parts of the pool. The inspector's kit registered 0.76, 1.05, and 0.66 ppm in the same three pool locations. The ship retrieved a third test kit from a different manufacturer and a crew member with boots on collected three samples from the deeper water section of the pool, since the original three tests could only be taken from the shallower water in the wading area bordering the pool. The ship's first kit registered a free chlorine residual of 0.63 ppm, the ship's second kit registered 0.95 ppm, and the inspector's kit registered 0.36 ppm. The forward pool was immediately closed and netted so the chlorine concentration could be increased. The pool was re-tested using the three test kits and all values were above 1.6 ppm.

Deck tiles in several different food service areas had missing and pitted grouting. This was noted on the previous inspection report. A plan to replace the grouting had been developed and several areas had already been replace. Some places identified in the plan that still needed to be addressed were the main galley potwash, main galley roast section in front of oven, and main galley dishwash area outside the stainless steel bin.
Potable Water-Production Dosing Station
A potable water hose tap at the bulkhead in this area was protected with an AVB backflow prevention device, but there was a shut off valve at the end of the hose. The valve at the end of the hose was removed during the inspection.
Recreational Water Facilities-Water Slide Free Halogen Residual
Staff were unaware that the free halogen residual for the water slide and the aft pool had to be kept at a minimum of 2 ppm due to the slide being classified as an activity pool. The water slide emptied into the aft pool and shared the same chlorination dosing system and compensation tank. Monitoring records for both the slide and the aft pool showed that free chlorine residual was never below 2 ppm. Manual records taken by the inspector and crew were above 2 ppm free chlorine residual. According to staff, the set points maintained by the ship are usually higher than the minimum requirements listed in the VSP Operations Manual.

The deckhead at the service side of the Pool Bar had openings between the deckhead slats, leading to open plenum space. The open spaces between the deckhead slats had a build-up of dust, especially above the liquor storage displays.

Several of the espresso machines have received splash panels since last inspection and according to staff, these units are next to receive them.

A steady water leak was observed along deckhead profile seams in several areas above the soiled staging area. According to staff, this was a result of condensation collecting on a poorly insulated cold water line above the deckhead.

One housefly was noted in this area. Overall area was clean and no other pest activity was observed.
Potable Water-Distribution System - Microbiological Samples
The monthly microbiological samples were being collected from various locations on the same decks over the months examined. These sample points should also be planned for a rotation of the decks as well as locations on those decks.
Children Area-Toy Cleaning
Dust, lint and other soil was found on a large variety of hard plastic toys that had been cleaned by immersion in a detergent sanitizer and returned to the children's area ready for play. Ineffective cleaning was noted in recesses, corners, and small holes in these toys.

Staff explained that these goods were placed on board two days ago. There was high traffic of personnel around the pallets exposing them to contamination.

Medical-Crew Acute Gastroenteritis (AGE) Reporting
The review of the AGE logs for the last 5 cruises revealed that 2 crew members had onsets of AGE symptoms and worked while symptomatic before reporting to the medical center and 1 crew member worked while on isolation: (1) An assistant waiter had an onset of AGE symptoms on 16 June at 07:00, but did not report to the medical center until 18:21 that same day. According to her time card, she worked from 06:41 to 09:26 and from 16:05 to 18:38; (2) An assistant housekeeping steward had an onset of AGE symptoms on 5 June at 01:00, but did not report to the medical center until 19:10 that same day. According to her time card, she worked on 5 June from 00:03-05:07; (3) A casino dealer had an onset of AGE symptoms on 9 June at 12:00, but did not report to the medical center until 10 June at 17:51. According to her time card, she was on 'medical off' on 9 June from 09:00 - 19:00, but worked from 10 June at 19:29 to 11 June at 02:29, during her isolation period. The staff was unaware of these discrepancies between the dates and times documented in the STARDOCS database and the time card system.

Housekeeping-Outbreak Prevention and Response Plan (OPRP)
The OPRP stated the medical center would be sanitized with 1,000 ppm bleach every hour or more to prevent cross-contamination, but did not indicate the surfaces which the disinfectant would be applied to. The plan stated food contact surfaces in the galley would be washed, rinsed, and sanitized with 150-200 ppm and left to air dry. The plan did not specify the disinfectant used and the contact time for this concentration.

Under normal operating conditions, the light intensity at the handwashing station in the Cafe Piazza and the Casino Bar was less than 110 lux. There was a bulkhead-mounted light at the handwashing station in the Casino Bar that would increase the light intensity above 110 lux. Staff was not aware this light needed to remain on during operation. These areas were open at the time of the inspection.

Medical-Acute Gastroenteritis (AGE) Surveillance Log
The electronic database used for collecting AGE illness information could not be exported into the standardized AGE surveillance log exactly as it appears in the 2011 VSP Operations Manual.

The electronic database used for collecting AGE case information used the previous VSP definition for an AGE illness case and did not consider diarrhea in excess of what was normal for the individual. A person with an underlying medical conditions was reported as an AGE case due to this database error.

The electronic database for AGE illness tracking did not include the ability to add symptoms when non-cases and cases were followed up to evaluate their condition daily. Notes were permitted to be added, but they did not detail symptoms, except to state the patient had more symptoms.

In many areas of the OPRP, the word 'frequently' was applied to the cleaning and disinfection of a hand contact surface. There were many food related areas where 1,000 ppm chlorine was specified for disinfection of refrigerator door handles, tongs, room service plates and trays, food and plate transport trolleys, ice scoops and all bar surfaces, provision hand contact surfaces, etc. Where Vital Oxide was specified to clean/disinfect, there was either no mixing ratio or no final concentration. In review of the product's label, there were specific mixing ratios or full strength listed for different applications and for destruction of specific pathogen types. In each section for a department there was no explanation of the product to be used on surfaces or the concentration and contact time for the product. In some cases an action was listed as 'if necessary close the pools or whirlpools,' or 'that in an outbreak the Hotel Director would determine if it was necessary to close the buffet to passenger self-service.'
Housekeeping-OPRP
There were no letters to notify current passengers of an AGE outbreak. Additionally, the letters to notify oncoming passengers of an AGE outbreak on the previous voyage were not available. Small toys in the child activity center were to be soaked in 1,000 ppm chlorine and rinsed with water. There were no written procedures to protect passengers from the respiratory or dermal hazards from the disinfection chemicals used in spray, wipe, and fogging activities.
Housekeeping-Public Toilet Rooms
There were no signs in the public toilet rooms advising users to wash their hands. Also, there were no signs advising the use of the door-mounted tissues to open the door when exiting the toilet rooms.

Recreational Water Facilities-Alkalinity and Clarity
Staff were not monitoring the swimming pools and whirlpool spas for total alkalinity or clarity.

The safety signs for the swimming pools and whirlpool spas did not include any of the statements required in the 2011 VSP Operations Manual.

There were no flow meters on any of the swimming pools or whirlpool spas. The turnover rates and bather load were not calculated for any swimming pool or whirlpool spa.

Each of the three swimming pools had two or more gravity drains greater than three feet apart constructed of a standard design and no alarms were installed. The suction drain covers on all of the whirlpool spas did not meet the requirements of the 2011 VSP Operations Manual and were not stamped accordingly.

The condensate collection pans were not visible from the access doors for the fan units. There was no fiber optic system available to properly inspect each condensate pan.

Food Service General-Galleys
Numerous of the two and three door undercounter reach-in refrigerators had one or more doors which did not seal tightly when shut, leaving a large gap near the bottom of the door and the frame of the refrigerator. This condition was noted throughout the main galley, crew galley, and David's specialty galley. Staff noted they are aware of the problem and have been replacing the warped refrigerator doors throughout the ship.
Galley-Appetizer Pantry
Two hotel pans of mixed bean salad, one hotel pan of char grilled vegetables, and one hotel pan of ratatouille salad were found inside walk-in refrigerator #11-07 with food temperatures of 45 °F. All were documented in the cooling logs as being prepared 20 August and no other temperatures were recorded to assure safety, so they were all discarded.
Galley-Main and Crew Galley
Due to some different thermometer values between food staff and the inspector an ice/water bath was set-up to check food thermometers in the main and crew galleys. There were only two food employee food probe thermometers that registered correctly at 32 °F, as did the inspectors. The 7 other food employee food probe thermometers registered 34, 28, 29, 18, 26, 30, and 25 °F, respectively. All those thermometers were immediately re-calibrated.

The previously cleaned slicer had brown food residue visible on the slicer back plate, near to the back side of the blade.

During active use of the flight-type conveyor dishwasher the spray from the rinse compartment was intermittent, cycling thru a full spray from all nozzles to no spray and back to full spray again.

During the 7-14 August 2011cruise, a linen valet had onset of reportable gastrointestinal illness (GI) symptoms and worked while symptomatic before reporting to the medical center. He had onset of GI symptoms on 13 August at 07:00, but did not report to the medical center until 14:45. His supervisor confirmed he worked 08:00 - 12:00.
Potable Water-Production Records
The production of potable water began on 6 April 2011 at 19:45 and ended on 7 April 2011 at 09:15. The dates and times of potable water production were also noted in the ship's NAPA electronic log. The manual chlorine concentration record for potable water production listed times of 20:10 and 23:00 on April 6th, then 02:00, 05:00, and 08:00 on April 7th, but there were no free residual halogen readings written for these times.

For all of the cooking equipment in this area, there were gaps around the pipes and drainpipes (water and grease) where they penetrated the upper panels in the undercounter technical compartments. This included the grills, the pasta cooker, and the fryers.

A passenger slipped on the wet deck and injured her left knee while walking on the Sun Deck (#10 portside amidships) of the C/S CARNIVAL PRIDE to disembark in Freeport/Nassau, Bahamas at 0630 in the morning. The fall caused her to sustain a fracture to her left knee cap. She was taken to the ship's medical facility for treatment.

US passenger onboard M/V CARNIVAL PRIDE lost his balance while transiting from the bathroom in his cabin lost his balance and fell fracturing his left femur while the vessel was underway in the Atlantic Ocean. Vessel returned to Baltimore, MD the following day and injured passenger disembarked the vessel and was taken to Baltimore Shock Trauma for treatment of a fractured left femur.

On July 24, 2014, Sector Baltimore received a call reporting an injury of a passenger aboard the vessel CARNIVAL PRIDE. On July 21, 2014, at approximately 0835, the passenger vessel CARNIVAL PRIDE (IMO 9223954) was underway on the Atlantic Ocean approximately 14 miles east of Hatteras Island, NC when a female passenger tripped and fell while walking by the photo gallery shop. The passenger received medical attention aboard the vessel and was not MEDEVAC. The passenger injured her left wrist and upper lip. The vessel continued scheduled route without any further incidents. No enforcement actions recommended.

On 03/07/14, SCC Baltimore received a notification from the CARNIVAL PRIDE that a passenger had twisted their ankle on 03/03/14. The twisted ankle was later diagnosed as a fractured ankle.

On February 11, 2013 while transiting on the Chesapeake Bay a passenger onboard the C/S CARNIVAL PRIDE was walking from the casino towards the "Atrium Lobby" on the second deck. She lost her footing and fell backwards onto the marble floor injuring her left hip. The passenger was disembarked in Port Canaveral on February 13, 2013 and transported for medical care via ambulance.

On 20 APR 2012, SCC Baltimore received a CG-2692, Report of Accident, Injury or Death via fax stating that a US Citizen aboard the Panama Flagged cruise ship, CARNIVAL PRIDE, died onboard the vessel while the vessel was at anchorage in Half Moon Cay, Bahamas on April 19, 2012. The female passenger just returned to the ship from a CCL recommended Snorkeling Tour when she suddenly collapsed and suffered cardiac arrest. She received CPR by life guards and was transferred to ships medical center via stretcher. She was pronounced dead on board at 11:45 am on 19APR2012 by the ship's physician.

On March 26, 2012, at approximately 0323 local time, the Master of the M/V CARNIVAL PRIDE (O.N. 9223954) notified the Sector Hampton Roads Command Center of a passenger who had suffered an apparent heart attack. Coast Guard assets were launched to effect a medevac from the vessel to a local hospital, where the patient was eventually declared deceased, with the cause of death to be cardiac arrest. As per MSM Volume V Chapter B7 guidance, this death case was downgraded to a data collection activity level of investigation after credible evidence (death certificate stating that cause of death was cardiac arrest resulting from a pre-existing known heart condition) indicated death from natural causes.

30Mar11: While the CARNIVAL PRIDE was moored port side to CT-5 in Port Canaveral, FL, the ship experienced sustained winds in excess of 70 knots directly on the port beam. This caused 8 mooring lines to part (4 forward and 4 aft). The failure of the mooring lines put the remaining strain on the lines attached to mooring capstans. The capstan brakes were overwhelmed and failed on most of these lines allowing line to pay out and causing the ship to drift off the pier. All three bow thrusters, both azipod propulsion systems, the starboard anchor and three assist tugs were utilized to control the ship until winds subsided. The CT-5 passenger gangway that was attached to the ship at the time that the lines parted was damaged and knocked off of its guide tracks. The ship was brought safely back alongside CT-5. There were no injuries or pollution reported or observed.

Sector Miami received notification from the Carnival Pride of a US citizen. male passenger, with cardiac arrest. Carnival Pride is 40 NM NE of Lake Worth Inlet headed to Nassau; LPOC was Cape Canaveral. Sector Miami briefed D7/Flight Surgeon. Flight Surgeon recommended MEDEVAC via Helo and SEC MIA concurred. Sector Miami briefed AIRMIA and determined Palm Beach Gardens Hospital to be the closest and best-fit hospital for the situation. Palm Beach Gardens Hospital can conduct open-heart surgery if need be. CGR 6608 conducted MEDEVAC of passenger and nurse is going with patient. Carnival Pride Doctor consulted with the Palm Beach Gardens Doctor via phone. CGR 6608 transferred patient to PBGH. Downgraded to Data Collection due to the incident is involving injuries resulting from a pre-existing medical condition and not from a marine operation. Once CG-2692 was received, it was determined that the passenger was actually a citizen of Canada. Per 46 CFR 4.03-1, this incident does not meet the definition of a reportable marine casualty (as pax was a Canadian citizen, the ship is not U.S. flagged, and the incident did not happen in navigable U.S. waters).

01Mar10: Received a notification in regards to a passenger on board the M/V CARNIVAL PRIDE possibly fracturing his left hip. MDO attended the vessel to conduct an on scene investigation, and pick up documentation applicable to the incident. Met with the STAFF CAPTAIN and SSO to determine the extent of the passenger's injuries. During the course of discussions it was determined that while in his cabin, the PASSENGER lost his balance, and fell on his left hip. Upon completion of mooring up at CT5, an ambulance disembarked the PASSENGER to transfer to Cape Canaveral Hospital. PASSENGER suffered a broken hip and underwent surgery.
