Federal health inspectors from the Centers for Disease Control and Prevention inspect cruise ships twice a year, checking everything from kitchen equipment to dishwasher temperatures, and grade ships based on what they find. A score below 86 is considered failing. Cruise ships are also required to report all illness outbreaks affecting more than 3% of passengers or crew to the CDC. Inspections and outbreaks from 2010 to the present are shown below.

Two more live fruit flies were found on the deckhead directly above the waiter station labeled for soiled items, and also at the aft end of the port pantry beverage counter.
Dining Room-Gold Pearl - Side Stands
The small square side stands were in use at breakfast for clean storage of food-related items in the cabinet below the stone top and between use storage of either soiled tableware or staging of filled beverage pitchers. The deck surrounding each of the small square side stands was carpeted and not coved.

A number of the deckhead lights over both the port and starboard dining room pantry beverage stations were not working. The resulting artificial light levels were below the minimum 220 lux.
Galley-Undercounter Reach-In Refrigerators
In several areas of the main galley one or more doors on the two and three door undercounter reach-in refrigerators did not seal tightly to the door frame when closed, particularly in the lower half of the refrigerator door. The gap present at the lower door was large enough to pass a set of folded papers from the outside to the inside of the refrigerator. The following areas and refrigerators with this condition are as follows: hot galley center - 3.4028 and 3.4046, appetizer pantry - 3.4107B and 3.4107A, and pastry - 3.4222 and 3.4210.

Several clean cutting boards on the clean storage rack had both front and back surfaces filled with raised thin plastic fibers that could be removed with little effort and appeared to have been created when a tool was used to re-surface each of the boards. A staff specialist was brought in to both evaluate the condition of the cutting boards and to ensure they were removed to be repaired again.

During active use of the conveyor pot wash machine, two of eleven spray nozzles were clogged in the final rinse upper spray arm. Staff corrected this during the inspection.

Staff stated the water was caused by the high humidly and subsequent condensation above the deckhead.

The light intensity for the lower level aft port and starboard beverage station was less than 220 lux. The light intensity behind and around the counter-mounted juice machines was less than 110 lux.
Medical-Acute Gastroenteritis (AGE) Reporting
A food employee reported to the medical center on 16 June having experienced 2 episodes of diarrhea that were not normal to him, having no underlying illness, and was deemed nonreportable by the medical staff.
Medical-Isolation of Symptomatic Crew Member
A nonfood employee was isolated for less than 24 hours. The employee reported to the medical center on 12 July at 1850 having experienced diarrhea and vomiting. On a follow up consultation on 13 July at 1744 the medical staff found that the employee was still unhappy, having experienced three more vomiting episodes since the last visit. The employee was deemed fit for duty and out of isolation on 14 July at 0908.
Potable Water-Dialysis
There were no plumbing connections with appropriate backflow prevention for passengers with dialysis needs. Staff explained that when there are passengers with dialysis needs they bring their own dialysis machines and use the hose connections in the cabins for people with disabilities.
Potable Water-pH Buffer Solution
During active bunkering, the pH was measured by the inspector at 6.8 and by the staff at 7.6. The calibration of the staff's pH pen was checked and it seemed to be accurate. However, it was not clear whether the pH buffer solutions being used to calibrate the pH pen were reliable. The bottles of buffer solution did not have an expiration date. Staff had a letter from January 2014 from the manufacturer stating that the solutions did not have a set expiration date but rather their reliability depended on the storage and handling conditions. This letter was requested by the ship's staff after a similar note was written on the previous VSP report of January 2014. The pH pen was being used to measure the pH values for all potable water measurements.

Galley-Potwashing
The wash temperature of the operating potwashing machine was 127°F rather than specified 150°F. The machine was checked again before the team left the crew galley and the wash temperature had returned to 152°F.

The outside chute from the ice maker to the ice storage bin was covered in slime mold. The refrigeration technical team disassembled the machine and thoroughly cleaned it after discarding all of the ice in the ice bin while the inspection team was still in the crew galley and mess areas.

About an 18-inch band of tape was used to close the door on the bone crusher as an adjunct to secure the door during the machine's operation. The tape creates a difficult to clean non-food contact surface. It was removed while the inspection team was still in the area.

Over 35 trolleys of soiled dishes completely filled the out of service forward dishwashing area. The soiled dish trolley's extended into toaster and milk dispenser area and were immediately adjacent to these pieces of food dispensing equipment.

Beef for the lunch service was being cooked on a recessed flat grill that still had egg residue from the breakfast service inside the cooking area and around the top of the grill. The cooking operation was suspended and the beef was discarded by the ship's staff.

Three trolleys of clean dishes and four empty clean dish trolleys were found in the unfinished transportation corridor adjacent to the dishwashing area to keep from overloading the dishwashing area. An empty clean dish storage area was noted across from the dishwashing area that would have accommodated these clean trolleys.

A non-food employee who experienced 2 episodes of diarrhea on 15 December, was not listed as a reportable case even though the medical records indicated that the diarrhea episodes were above normal for the individual. The employee was isolated 24 hours from the last symptom.
Potable Water-Micrbiological Testing
The incubator being used for analyzing water samples was operating at 30°C instead of the required 35°C (+/- 0.5°C) as specified in the instructions for the test reagents being used. A bunkering water sample test was being administered in the incubator during the observation. The temperature in the incubator was increased.

The faucet to the hand wash sink had a threaded hose connection with no backflow prevention devices. No hose was connected to the faucet.

Three grey plastic bus tubs full of food, soiled dishware, and coffee pitchers were stored directly on the deck. According to staff, these items were removed from the passenger cabins and stored in this location until room service had time to remove.

Medical-Antidiarrheal Medication Dispensed
For all passenger and crew member acute gastroenteritis (AGE) reportable and non-reportable cases, the print-out from the STARDOCS medical surveillance system indicated the patient was issued antidiarrheal medication, even though medical staff clearly marked 'no' in the appropriate box in the STARDOCS medical surveillance system.

According to the STARDOCS medical surveillance system, a Team Waiter had his last AGE symptoms on 12 August at 23:00. A medical staff wrote his last symptom was on 13 August at midnight. This crew member was isolated 11 August at 17:51 to 15 August at 08:08. This was not a complete 48 hours of medical isolation following his last symptom.An Assistant Waitress had an onset of AGE symptoms on 12 July at 16:30. When the medical staff conducted their follow-up interview on 14 July at 16:03, this crew member indicated she was still experiencing symptoms. According to the STARDOCS medical surveillance system, her isolation started on 12 July at 16:52 and ended on 15 July at 16:55. The isolation start time was when she reported to the medical center, and not the time when she was symptom-free.

On the print-out from the STARDOCS medical surveillance system, the date of a patient's last symptom was often difficult to determine because the last digit was cut off. For example, a crew members last AGE symptoms were on 14 August, but the print-out indicated 1 August,

When interviewing an AGE patient's close contacts, not all those who shared the cabin toilet room with the symptomatic patient were included.

Also, a patient was often not isolated for the appropriate 24 or 48 hours symptom-free. For example, a Team Waiter had his last AGE symptoms on 13 August at midnight, but his medical isolation ended on 15 August at 08:08. Also, medical staff listed patients as non-reportable cases, even though a patient experienced AGE symptoms that were not normal for him/her or experienced AGE symptoms that met the case definition as a reportable case. For example, a Hairdresser had one episode of diarrhea, which she indicated was above normal. She was listed as a non-reportable case. A Team Headwaiter had two episodes of diarrhea, was listed as a non-reportable case, but there was no notation of an underlying illness or these condition being above normal for him. A Photographer had two episodes of vomiting and headache and was listed as a non-reportable case. Also, medical staff was not conducting interviews with all individuals who shared toilet facilities with the AGE patient.

On 11 November 2012, corporate staff sent out procedures for fecal/vomit accident response. These procedures included (1) wash with HB Quat or a similar product; (2) rinse with fresh water; and (3) sanitize surfaces with 200 ppm chlorine. Since the last inspection, there were 8 accidents, all of which were vomit. The write-ups for each of these included sanitizing surfaces inside the pool, ladder, overflow channel drain, etc. with the 200 ppm chlorine, but there was no contact time for this solution.

In reviewing the complete case definition both from their electronic database and the VSP Operations Manual it seemed the staff applied what is normal for the individual only when an underlying medical condition was found and not in all persons reporting diarrhea.

An assistant waitress had onset of GI illness symptoms on 5 April and was isolated in her cabin. On 6 April the original GI symptoms had resolved, but there was fever in the afternoon of the 6th. Food Service General-Ice Machines
Per staff, the ice making components of the ice machine were cleaned with a chemical and rinsed with water. The manufacturer's instructions stated to call the service company to clean the ice machine. Staff will contact the manufacturer to determine the cleaning and sanitizing procedures.

All access openings in bulkheads at ends of enclosed superstructures shall be fitted with doors. The means for securing these doors weathertight shall consist of gaskets. Door located on deck 09 aft portside directly by the pool bar is not weathertight.

Any required fixed fire detection system shall be capable of independent operation at all times. The detectors shall be designed to automatically restore the system to normal surveillance a a predetermined time. Vessel is fitted with a smoker detector disabling device that is not capable of restoring the system automatically.

Personnel Casualties : 1 injury
On 22 September 2014 at approximately 2030, a passenger on the cruise ship CARNIVAL SPLENDOR was injured when he stepped on a metal strip attached to a set of stairs. The passenger sustained a spontaneous rupture to the left achilles tendon, a "moderate" injury per Marine Safety Manual Volume V policy; not requiring incident causal analysis.

Station Port Canaveral received phone call from Port Canaveral Police Dispatch stating that a crewmember had died onboard the CARNIVAL SPLENDOR while moored in Port Canaveral. Vessel rep stated that the crewmember had become sick immediately after arriving at the vessel 5 days prior and was being seen by the shipâs Doctor. Body taken by the Brevard County ME. Pending autopsy results for cause of death. 19MAR14 Received Medical Examiners Report; Cause of death was BRONCHOPNEUMONIA. Changed INV to DATA Collection due to Pre-existing Medical Condition.
