Federal health inspectors from the Centers for Disease Control and Prevention inspect cruise ships twice a year, checking everything from kitchen equipment to dishwasher temperatures, and grade ships based on what they find. A score below 86 is considered failing. Cruise ships are also required to report all illness outbreaks affecting more than 3% of passengers or crew to the CDC. Inspections and outbreaks from 2010 to the present are shown below.

On the passenger side, a food employee was dispensing food to passengers. This food employee over reached beyond the protection of the sneeze guard, sticking his head and part of his upper body under the sneeze guard and above the now exposed food. The food employee was observed doing this twice. Managers in the area provided an additional food employee in the food workers side of the buffet line to assist with food dispensing.

Food became unprotected when a food employee was dispensing food to passengers from the passenger side, sticking his head and part of his upper body under the sneeze guard and above the food. This was corrected.

Room Service-Deck 2

Soiled plastic beer buckets and metal wine buckets were found stored directly on the deck below the soiled dish return table. The main automatic dishwasher was temporarily out of order and caused a large backup of soiled items in the return area. The beer buckets and wine buckets were immediately moved to the soiled table. The other soiled tableware and utensils were moved to another dishwashing area while the dishwashing machine was repaired.

A slotted fastener was in the food splash zone on the panel around the dispensers on each of the two coffee machines. This was corrected.

The light intensity could not be raised to at least 220 lux at some sections of waiter stations #49 and #50.

Galley-Cabanas Dishwasher

The wash solution for the glassware machine did not reach the machine's data plate required 160°F during active use. The machine's wash gauge read 155-160°F and manual readings inside the wash tank taken with the inspector's thermocouple ranged from 155-158°F. Staff measured the temperature of the wash to be 159°F by using a thermocouple with an extension cable and running it through the active wash compartment. Staff later reported that they were able to increase the wash temperature after the discovery of a rubber material in the wash tank. According to staff, once the rubber material was removed the wash temperature was able to reach 160°F.

The electrical wire of the microwave was lying on the counter. This was corrected on site.

Food Service General-Espresso Machines

The area around the water level indicator on many of the espresso machines was soiled with more than a day's amount of dust and residue. This was noted in the beverage station area of Remy's, Concierge Lounge, Vista Cafe, Cove Cafe, and Bon Voyage Bar.

The electrical wire of the water purifier was lying on the counter. This was corrected on site.

Housekeeping-Outbreak Prevention and Response Plan (OPRP)

Under the Youth Activities section, it was indicated that some surfaces would be disinfected with 1000 ppm chlorine and allowed to air dry. However, there was no contact time for the chlorine.

Galley-Royal Palace

Small counter-top combination steam ovens #17862 and #17863 had frayed and difficult to clean thermistor sensor cables in the interior cooking compartment. Both of these units were remove from service while the inspection team was still in the area.

Buffet-Starboard Dry Storage Locker

Different types of dishware and food service utensils for the buffet were stored on galvanized metal shelving. Some of the metal shelves were painted and the paint was chipping and peeling off in many locations.

Buffet-Starboard Waiter Station #3

The deckhead light above the handwash station was burnt out resulting in less than the required 220 lux of light.

Dining Room-Remy - Menus

The dinner menu for Remy's had asterisks at animal derived foods that could be served raw or undercooked and a consumer advisory statement at the bottom of the menu, however, there was no asterisk at the consumer advisory statement needed to disclose the meaning of the asterisk. The ship began to print new menus during the inspection.

Bar-Cove Cafe

Mold growth was observed on the soda gun nozzle where the removable plastic cap attached. Mold was also observed on the inner side of the plastic removable nozzle cap. This was cleaned before the inspector left the area.

Pantry-Bon Voyage

The chilled water lines to the ice machine were not labeled as non-potable.

Preparation Room-

There was no cooling log documentation for a large bin of cut cabbage stored with the lid on in the walk-in cooler. The cabbage was stored in a deep plastic bin holding approximately 15 inches of cut cabbage. Temperatures taken by the inspector were below 41°F.

In the Remy Galley, the light intensity was less than 110 lux behind and around the combination oven. In the Palo Galley, the light intensity was less than 110 lux to the right of the combination oven.

Integrated Pest Management-Active Monitoring Records

On some of the night time active monitoring records, the times of the inspections were not documented.

Potable Water-Bunkering Halogenation / pH Adjustment

The pH of the bunker water after treatment was measured at 7.9 - 8.1 with the inspecor's meter. The analyzer and the ship's pH meter measured the pH at 7.4. The ship's pH meter was checked against the ship's pH 4.0 and 7.0 standards and found to be 0.5 pH units out of calibration.

Buffet-Crew Mess

A bowl of apples and a basket of bread rolls at the self-service line did not have dedicated serving utensils.

Room Service-Pizza Oven

The high temperature gasket to the top pizza oven door was frayed in several areas and fell apart when touched by the inspector. The gasket was replaced during the inspection.

Galley-Deck 2 - Starboard Enchantment Garden Beverage Station

The underside of the dispensing nozzle to the counter mounted hot chocolate milk dispenser was soiled with old sticky chocolate residue of more than a day's accumulation. The dispenser was cleaned before the inspection team left the area.

Buffet-Cabana's STBD Cold Line

The handles of the serving utensils being used to plate the ready to eat cut melon and watermelon were stored in contact with the food located in the undercounter cold refrigerator on time control.

Buffet-Time Control Plan

Counter tops being used as work areas for food on time control during a cooking process were not identified on the time control plan. For example, there was a hotel pan of pancake batter at the pancake cooking station, but the plan did not specifically indicate that workstations would operate on time control. The pancake batter did have a time control discard label.

Buffet-Outside Cabana STBD

There was an active sanitizing bucket that had less than 10 ppm chlorine residual located at the dirty waiter station nearest the passenger walkway to the inside Cabana buffet.

Galley-Date Marking

A cooked container of diced chicken had a discard date which was calculated from when the chicken was diced, rather than when the chicken was cooked. The cooling logs indicated that the chicken was cooked and cooled on 18 October and then diced on 20 October. The date marking reflected a discard date of 25 October as per the 5 day discard rule implemented by the ship.

The counter mounted espresso machine had two slotted fasteners at the coffee ground/water juncture.

The base of the blender unit where the mixer attaches was soiled with a brown sticky residue. This was corrected during the inspection.

Potable Water-

The list of air gaps for the RWF fill lines had inspection dates from October 1, 2012 to October 1, 2019. Also, the fill lines for the RWFs were not identified as the plumbing component. Only the compensation tanks were identified in the description. The inspection dates for the rest of the air gaps were indicated from October 1, 2019 to October 1, 2029. Finally, there were two backflow prevention devices listed for the private whirlpools, but one device was listed as a Watts 9D and the other was listed as a Watts 9DM2. Upon inspection, both devices were Watts 9DM2. Also, the location of the whirlpool was not exact and just stated they were nearby the Chief Engineer's cabin and nearby the Captain's cabin.

Recreational Water Facilities-Shocking Records

The fwd and aft Rainforest whirlpools and the Villa port and starboard whirlpools had a chlorine residual of 1.0 PPM at the completion of shock halogenation from September 9, 2012 to October 20, 2012. According to staff, the completion shock chlorine levels were 10.0 ppm.

Galley-Royal Palace Hot Serving Line

Condensate from an excessively hot bain marie was collecting on the bottom of the pass through shelf. It was not dripping and no food was impacted.

Galley-Enchanted Garden Beverage Station

The hand towel dispenser was out of towels. These towels were immediately replaced. Three other hand towel dispensers in the immediate area were full.

Preparation Room-Vegetable Preparation

Slight dried soil was noted on the back of a previously cleaned food slicer blade.

Provisions-Dry Stores

A cardboard carton of dried mushrooms and its inner plastic lining were damaged exposing the carton contents. This box was immediately discarded.

Potable Water-Cabin Toilets and Showers

The list of backflow prevention devices for the passenger and crew cabins indicated two devices with the maker as Watts/RROHE and the type as Oras. The list did not indicate the plumbing system component to which the devices were installed. It was determined one vacuum breaker was installed for each toilet and shower. Staff showed an Oras 261-031 device, which matched the devices installed for the toilet and shower.

Potable Water-Pressure Relief Valve

The list of backflow prevention devices did not include the air gap for the pressure relief valve located next to mineralizer #3.

Potable Water-Steam Room

The list of backflow prevention devices did not include the two devices for the boilers in the steam room.

Recreational Water Facilities-Safety Signs

The safety signs for the Mickey's Slide and Aqua Duck did not include the information required in the 2011 VSP Operations Manual.

Bar-Cove Cafe

A spray container of air freshener was found stored in the back bar undercounter trash cabinet. Staff immediately moved the container to the cleaning locker.

Aluminum foil was used as a shelf liner in the carbonator cabinet. Staff immediately removed the foil.

Milkstone had formed on the inside of the metal frothing cup.

The boiler drain was soiled with old food residue.

The temperatures at the three faucets of the handwashing sink outside the dishwash area were measured at approximately 125°F. The users could not adjust the temperature. This was corrected.

The backflow prevention device for the coffee machine closest to the bulkhead was leaking.

Buffet-Officers Mess

There was no sign posted indicating that cold smoked salmon was served raw or a consumer advisory statement at the specific station where cold smoked was served. There was a consumer advisory statement at the entrance to the buffet.

Food Service General-Ice Cream Machines

The chilled water lines for the ice cream machines were not uniquely identified. This was in progress.

Galley-Animators Dishwash Area

Two soiled dish rack trolleys were stored against and touching the front side of an ice machine. The trolleys were moved.

Medical-Acute Gastroenteritis (AGE) Reportable Case Definition

The AGE reportable case definition for diarrhea episodes did not include what is above normal for an individual.

Medical-Acute Gastroenteritis (AGE) Log

From the last USPHS inspection until December 7, 2011, the AGE log did not contain a specific case identification number and did not list the required items in the correct order as stated in Annex 13.2.2 in the 2011 VSP Operations Manual. A new AGE log that meets the USPHS requirement was implemented on December 8, 2011.

Housekeeping-Outbreak Prevention and Response Plan (OPRP)

There were no procedures for informing passengers and crew of an outbreak in the OPRP. Also, there were no specific procedures to protect the passengers from dermal and respiratory exposures to disinfectants in the OPRP.

Children Area-Oceaneer Lab Toilet Rooms

The girls and boys toilet rooms are used by children 3-12 years old but the handwashing sink was 24 inches above the deck and the toilet was 15 inches above the deck. Step stools were not available in these areas.

There was no disinfectant located in the diaper changing station.

The two outer sinks in the boys and girls club toilet rooms were 24 inches from the deck and the toilets were 15 inches above the deck. Step stools were not available in these areas.

There was no time control plan located in the Oceaneer Club for the food served in that area. A time control log was available in the club with food temperature measurements and times for when food was received at the club and discarded. All food was discarded in less than 4 hours.

There were no tissues or towels available to exit the men's or women's changing rooms and there was no sign available advising users of the toilet facilities to use hand towel, paper towel, or tissue to open the door. The exits were not hands free.

Bar-Cove Cafe and Currents Time Control Plans

The time control plans at the Cove Cafe and Currents did not list the set-up and discard times. Temperature logs were at the outlets and included the set-up and discard times. All food was discarded in less than 4 hours.

The inside of the previously cleaned Michelob beer tap line was soiled with a residue.

The inside of the three beer tap lines were soiled with a residue. The beer taps had not been used yet today and had not been cleaned in almost two weeks.

The lock mechanism for the walk-in refrigerator door D-33-1-009 had been out of operation and missing since December 8, 2011. There were two holes in the door and the missing latches created an opening within the door leading to a void space. This missing lock was not impacting the temperature of the unit or the food within it.

Integrated Pest Management-Active Surveillance

The active surveillance logs written by the IPM contractor did not include the follow-up inspection results. The sighting logs conducted by staff did not include the time of the inspection and did not always indicate what type of pests were found. In some cases a pests was noted as 'bites' in the log and even though a spray was used, the actual pest was not identified in the log.

Recreational Water Facilities-Fecal Vomit Accident Plan

The chlorine residual and contact time for a loose stool accident had a CT value of 9,600, new scientific research recommends a CT value of at least 15,300. (New requirement 2011 VSP Manual)

There were Watts 9D backflow prevention devices connected to the potable water lines in the pool and spa machinery rooms. These devices had plastic tubes connected to the vents to direct any possible water leak to a common drain. However, some of these plastic tubes were sloped upwards not allowing for easy drainage and identifying malfunction of these devices.

There were green and yellow residues that appeared to be mold on the lights fixtures in the rock area of the decorative water fountain.

Four of six trolleys used for linen/glasses/supplies for guest rooms that were checked on Decks 2 and 8 had racks of clean glasses that were not adequately protected from potential contamination. Partially consumed packages of food, bottles of water, sunglasses with straps and other items were found stored directly on the clean glasses.

Buffet-Lido Port Soda Cabinets

A squeegee, dustpan, and small broom were stored under the cabinet. No food or single use items were impacted. These were removed.

The outside of the interior plastic panel on the upper compartment of the ice machine was soiled.

Seven stacks of clean plates were stored on top of waiter station 20800. The plates were not covered or inverted. The dining room was not in use at the time of the inspection.

The draft beer tube was draped on the bottom of the technical compartment below the speedrail.

Dining Room-Royal Palace Waiter Station

Clean folded napkins were stored in the bottom shelf of the soiled section of the waiter station in the private room of the dining area and in the waiter station directly adjacent in the main dining room area. The shelves above the clean napkins were designated for soiled items. There were no soiled items in this area at the time of the inspection. A clean linen storage room was located nearby.

Galley-Deck 2 Bell Box

An excess amount of steam and condensation was collecting on the deckhead and dripping on the deck of the soiled side of the Hobart rack conveyor warewashing machine.

Galley-Enchanted Garden Port Side

Five previously cleaned white serving dishes were soiled with food debris and stored in the clean dish storage cabinet. The dishes were immediately returned to the warewashing area to be cleaned.

Galley-Pastry

Food items such as sugar, vanilla paste, and chocolate pieces were stored in an undercounter storage cabinet along with a clipboard and a stack of records.

Steam from combination oven 17807 was condensing on the deckhead and side of the exhaust hood and dripping to the deck. The oven was in the cleaning cycle.

Three previously cleaned coffee carafes at waiter station 16490 were wet inside. They were stored upright, impeding air drying.

The lighting was less than 220 lux at the handwash station and adjacent counter. This was an item from construction and, according to the staff, a light will be added by the ship yard within the next two weeks.

Medical-Gastrointestinal Illness Reporting by Food Employees

For the cruise dated 24-27 February 2011 there was a food employee (cook) that did not report to medical when gastrointestinal illness (GI) symptoms began. The first GI symptom occurred in the morning of 25 February 2011, but the cook did not report to medical until the afternoon of 26 February 2011. For the cruise dated 20-24 February 2011 there was a food employee (cook) that did not report to medical when GI symptoms began. The first GI symptom occurred in the evening of 18 February 2011, but the cook did not report to medical until the morning of 20 February 2011. According to the medical staff, these crew members were counseled and the medical staff followed up with their supervisors.

The water temperature at the handwash sink was over 125°F and the temperature could not be adjusted by the user. The area was not in active use. This was corrected.

Unknown location of theft. Could of possibly occurred on the ship. Victim thinks she may have left her rings onboard the Dream and didn't realize they were missing untill she got home. Sail date was 10/20 to 10/24, report was filed on 11/14/13. Forward to Detective.
